脉络膜黑色素瘤的影像学观察

Evaluation of Imaging Techniques for the Diagnosis of Choroidal Melanoma

目的 对比分析不同影像学方法对脉络膜黑色素瘤的诊断价值。方法 用ICGA、B超 (CDFI)、CT和MRI对2 3例经病理证实的脉络膜黑色素瘤影像表现进行分析。结果 术前ICGA、B超 (CDFI)、CT(增强CT)和MRI检查分别为12、2 2 (5 )、2 0 (11)和 5例。诊断为眼内肿瘤 :ICGA、B超 (CDFI)、CT(增强CT)和MRI检查分别为 3、2 1(4 )、19(10 )和 1例 ;诊断为脉络膜源性肿瘤 :ICGA、B超和CT分别为 3、1和 1例 ;与病理诊断完全吻合 :ICGA、CT和MRI分别为 6、1和 4例。结论 ICGA有助于屈光间质清晰的脉络膜黑色素瘤的诊断。B超和CT检查能够显示病灶的大小、形态等 ,可初步区分肿瘤的良恶性。

Objective To evaluate the imaging techniques for the diagnosis of choroidal melanoma. Methods Twenty-three cases which were confirmed by pathologic examinations were collected,and their data of ICGA,Ultrasonography B,CT and MRI were analysed. Results Before operations,the examination of ICGA,Bmode ultrasonography,CDFI,CT,enhanced CT and MRI were performed respectively in 12,22,5,20,11 and 5 cases,among which 3,21,4,19,10 and 1 cases were diagnosed as intraocular tumor by ICGA,Bmode ultrasonography,CDFI,CT,Enhanced CT and MRI,3,1 and 1 case were determined to be tumors derived from choroid by way of ICGA,ultrasonography and CT,6,1 and 4 cases had the diagnosis of choroidal melanoma according to ICGA,CT and MRI. Conclusion ICGA is valuable for the diagnosis of choroidal melanoma in patients with clear refractive media.Ultrasonography and CT can differentiate benign from malignant primarily by revealing the size and shape of the tumor.MRI can reveal the characteristic signals.
