围产期孕产妇促红细胞生成素(EPO)水平变化 被引量:1

Serum erythropoietin and its relationship to iron status markers in the third trimester and early postpartum period

摘要 目的 检测围产期促红细胞生成素 (erythropoietin ,EPO)浓度变化 ,探讨EPO与血红蛋白浓度 (Hb)之间的相关性。方法 用放射免疫法测定健康非妊娠妇女 15例 ,正常妊娠晚期孕妇 2 7例 ,妊娠晚期合并贫血孕妇 32例及产后7天内合并贫血产妇 15例的血清EPO和血红蛋白浓度。结果 上述 4组EPO水平分别为 (9 85± 38 5 2 )U/L、 (14 4 2±6 80 )U/L、 (99 82± 78 70 )U/L、 (5 9 5 0± 38 5 2 )U/L。妊娠合并贫血的孕妇及产后贫血的产妇EPO水平明显高于非妊娠妇女及正常妊娠晚期孕妇 ,P<0 0 1。四组血清EPO与Hb呈相反关系。结论 妊娠晚期及产后早期合并贫血时EPO水平明显升高 ,且对贫血有较强的反应。 To detect the changes in concentration of serum erythropoietin(EPO)in the third trimester and the relationship between EPO and serum ferritin(SF)or transferrin(Tf).Methods: Using a radioimmunoassay serum EPO concentrations were measured in 15 healthy nonpregnant women,27 normal late pregnant wowen,32 anemic women with late pregancy and 15 anemic women with early postpartum. SF levels were also measured by a radioimmunoassay and Tf levels were measured.Results:(1)Serum concentrations of EPO were significantly higher in late pregnant women with anemia(99 82±78 70)mu/ml than those in nonpregnant women(9 85±3 09)mu/ml,P<0 01 and also than those in normal late pregnant women(14 42±6 807)mu/ml, P<0 01.The results were the same in early postpartum women with anemia(59 50±38 52)mu/ml.VS,(14 42±6 80)mu/ml,P<0 01,and VS(9 85±3 09)mu/ml,P<0 05.(2)The serum EPO levels of subjects were negative correlated with their (Hct)% in four groups.(3)The serum EPO levels in pregnant women were inversely correlated with their SF levels(r=-0 581,P<0 05),while they were positively correlated with Tf levels(r=0 297,P>0 05)where SF≤20ng/ml,and there was also positive correlation between serum EPO and SF(r=0 475,P<0 01)or Tf(R=0 121,P>0 05)if SF>20ng/ml.Conclusions:We conlude that serum EPO concentration sinificantly increased in late pregrant women with anemia and in early postpartum women with anemia.The variation of EPO is closely correlated with transferrin and ferritin in the third trimester.
