急危重症患者的经颅多普勒超声应用研究 被引量:2

摘要 目的 :客观评价神经内科急危重症患者的脑血流动力学变化与临床预后的关系。方法 :以经颅多普勒 (TCD)超声仪动态监测急危重症 4 5例脑血流动力学参数。结果 :4 5例重症患者存活 17例 ,其中持续性植物状态 2例 ,死亡 2 8例。脉动指数 >2 .0 ,大脑中动脉平均血流速度 <2 8cm/s者均死亡 ,脑死亡型组和过度灌注型较颅内压增高型的病死率差异具有显著性。结论 :TCD监测急危重症患者脑血流变化规律 。 Objective: To achieve an objective evaluation of the relationship between hemodynamic changes and the clinical prognosis in patients with critical clinical conditions. Methods: 45 patients with critical clinical conditions were investigated by Transcranial Doppler (TCD) dynamically. Results: Among the 45 patients, 17 patients survived, including 2 patients in vegetative state, 28 patients died. The clinical manifestations and prognosis were obviously related to the minimum effective hemodynamic perfusion observed by TCD examinations. Conclusion: TCD monitoring of the hemodynamic changes for patients with serious clinical conditions has significant values in clinical therapy and prediction of prognosis.
