摘要
目的 :采用黄色背景光蓝闪光刺激ERG(blue on yellowflash ,ERG)方法 ,探讨非渗出性年龄相关性黄斑变性(AMD)的短波长敏感视锥细胞通路、视杆细胞系统以及双谷a波的改变特征。方法 :采用AVES 80 0 0型视觉电生理检查系统 ,在Ganzfeld刺激器上设置黄色背景光 (5 70nm)和蓝色刺激光 (45 0nm) ,检查程序及参数参照ISCEV标准。对正常人 (40眼 )和非渗出性AMD(40眼 )进行观察 ,记录视杆细胞反应、最大反应和视锥细胞反应。结果 :与对照组比较 ,非渗出性AMD的双谷a波数减少 ,视杆细胞反应b波振幅降低(P <0 .0 1) ,最大反应b/a比值异常 (P <0 .0 5 )。视锥细胞反应b波振幅下降 (P <0 .0 0 1) ,长波长敏感视锥细胞 (L cone)和中波长敏感视锥细胞 (M cone)活动受到明显压抑 ,短波长敏感视锥细胞 (S cone)活动得到充分反映。结论 :黄色背景光蓝闪光刺激ERG可以对视杆细胞系统和短波长敏感视锥细胞通路进行检测 ,非渗出性AMD的短波长敏感视锥细胞通路、视杆细胞系统和b/a比值异常 ,双谷a波数减少。
Objective:To investigate changes in the pathways of short wavelength sensitive cones,the rod system and the two trough a waves when blue on yellow flash ERG is used on patients with non exudative age related macular degeneration.Methods:An AVES 8000 was used with a yellow light background(570 nm) and blue stimulating light(450 nm) setting on a Ganzfeld stimulator. Exams and parameters were performed according to ISCEV standards. Rod and cone responses and maximum responses were recorded for normal subjects(40 eyes) and non exudative age related macular degeneration patients(40 eyes).Results:Compared to controls,the number of two trough a waves was reduced in AMD,b wave amplitudes of rod responses were lower(P<0.01) and the b/a ratio for maximum responses was abnormal(P<0.05). In the AMD group,b wave amplitudes of cone responses decreased(P<0.001),and the activity of long wavelength sensitive cones(L cone) and middle wavelength sensitive cones(M cone) was clearly restrained with an abundant reflect of short wavelength sensitive cones.Conclusion:Blue on yellow flash ERG can be used to examine activity in both the rod system and S cone pathway. The S cone pathway,rod system and b/a ratios are abnormal in non exudative age related macular degeneration with a decrease in the number of two trough a waves. Blue on yellow flash ERG is valuable in the clinic.
