摘要
背景与目的:食管癌高发区食管癌患者的头发、血清内Zn、Cu等微量元素有异常改变,但在不同病变食管粘膜内的含量有无变化、与p53和增殖细胞核抗原(proliferatingcellnuclearantigen,PCNA)的表达有何关系尚不清楚,本文就食管粘膜内微量元素含量、p53突变及PCNA表达与食管病变的关系作一探讨。方法:151例食管活检标本分为正常、慢性炎、非典型增生与早期癌四种类型,以X-射线能谱仪进行微量元素定量检测,以S-P免疫组化法检测PCNA表达及p53突变状态。结果:正常上皮、慢性炎、非典型增生及早期癌中Zn、Se、Mo的含量分别为1.74±0.32、1.53±0.64、0.58±0.21、0.20±0.08;0.15±0.06、0.10±0.03、0.04±0.02、0;4.73±1.31、3.45±1.19、3.51±1.32、2.51±1.04,不同组织学类型间差异显著(P<0.05)。Cu/Zn、Ni含量分别为0.57±0.17、0.89±0.18、2.45±0.48、2.92±1.08;0.45±0.05、1.27±0.11、2.46±0.24、2.58±0.33,随着食管病变程度的升级含量渐增,差异显著(P<0.05)。p53、PCNA阳性表达在正常上皮与非典型增生及早期癌组内分别为0、46.15%、100%;31.19%、84.62%、100%,差异显著(P<0.01),且与微量元素含量显著相关。结论:Zn、Se、Mo、Cu。
Background &Objective:There were abnormal changes of trace element content in esophageal cancer patients hair and serum in the high r isk area.But it was still unknown that the relationship between p53and pr oliferating cell nuclear antigen(PCNA)expression and the trace element con tent in varied esophageal mucosa.This study was designed to probe the relationship of trace elem ent content and p53mutation,PCNA ex pression in esophgeal mucosa.Methods:Esophageal biopsy specimen of 151ca ses were divided into four groups(normal,esophagitis,dysplasia,an d early carcinoma ).The quantitative determination of t race element was performed was perfo rmed by using X-ray energy spectrome try and the detection of PCNA expression and p53mutation was performed by using S-P i mmunohistochemistry method.Results:The contents of Zn,Se,Mo,were 1.74±0.32,1.53±0.64,0.58±0.21,0.20±0.08;0.15±0.06,0.10±0.03,0.04±0.02,0;4.73±1.31,3.45±1.19,3.51±1.32,2.51±1.04;respectively in four gro ups.There was a significant difference in varied histological t ypies(P<0.05).The contents of Cu /Zn,Ni were 0.57±0.17,0.89±0.18,2.45±0.48,2.92±1.08;0.45±0.05,1.27±0.11,2.46±0.24,2.58±0.33;respectively,which increased gradually in pace with upgrad e of esophageal lesions,with signifi cant difference(P<0.05).The postive rates of p53and PCNA were0,46.15%,100%;31.19%,84.62%,100%respectively in normal esophag eal epithelium,dysplasia,and early carcinoma,with significa nt difference(P<0.01),and had significant correlation to trace element.Conclusion:The content variation of Zn,Se,Mo,Cu,Ni could be possessed of certain effect on p53mutation and PCNA overexpression of esophageal e pithelium in the high risk area.
