摘要 目的 探讨螺旋CT三维成像对脊柱病变的诊断价值。方法 对 2 8例典型脊柱病变先行X线检查 ,然后对兴趣区进行螺旋CT薄层扫描 ,工作站后处理 ,分别获得表面遮盖和多层面重建图像。结果 ①SSD和MPR对脊柱骨折和先天畸形的显示优于X线平片 ;②X线平片和SSD对颈椎间孔狭窄的准确性分别为 75 %和 92 % (χ2 =5 .96,P <0 .0 5 ) ;③螺旋CT能更准确地显示脊柱结核和肿瘤骨质破坏的程度和范围。结论 螺旋CT三维成像能更好地显示脊柱病变的病理改变 。 Objective To evaluate the value of the three dimensional (3D) reconstruction of spiral CT in the diagnosis of spinal lesions. Methods Twenty eight patients with the classical spinal lesions were examined by X ray. Then spiral CT scan with thin slice was performed in the interested region with post processing on the workstation. Surface shaded display (SSD) and multiple planner reconstruction (MPR) 3D images were obtained respectively. Results ① SSD and MPR was superior to X ray film in demonstrating spinal fracture and congential deformity. ② The accuracy rate of SSD and X ray film in the diagnosis of cervical intervertebral foramen narrow were 92% and 75% respectively (χ 2=5.96, P <0.05).③ SSD and MPR could correctly demonstrate degree and extension of the destroyed bone in spinal tuberculosis and tumors. Conclusion Spiral CT can correctly show pathoanatomy of the spinal lesions, and it is superior to X ray film in diagnostic value.
