带旋髂深血管骨膜骨瓣移植治疗青壮年股骨颈骨折

Treatment of Femoral Neck Fracture in Young Adults with Vascularized Osteoperiosteal Flap Pedicle of Deep Iliac Circumflex Vessels

目的 :探讨青壮年股骨颈骨折采用AO加压螺纹钉内固定 ,带旋髂深血管骨膜骨瓣移植的疗效。方法 :经髋关节S -P切口 ,股骨颈骨折行切开复位 ,3枚加压螺纹钉固定 ,带旋髂深血管蒂骨膜骨瓣移植于股骨颈骨折处 30例。结果 :随诊 2 9例 ,时间 1～ 8年 ,2 7例疗效良好 ,优良率达 93%。 1例股骨头缺血坏死 ,1例骨折不愈合。结论 :AO加压螺钉内固定加带旋髂深血管蒂的髂骨骨膜骨瓣移植术是治疗青壮年股骨颈骨折的有效方法 ,局部血供丰富 ,骨折愈合率高 ,减少了并发症的发生。

Objective: To explore the treatment effect of femoral neck fracture in young adults by AO compression screw bundle of fixation and osteo periosteal bone grafts pedicle of deep iliac circumflex vessels.Methods:30 cases were treated by three compression screws fiction and osteo periosteal bone grafts of deep iliac circumfluent vessels in the femoral neck fracture with Smith Peterson incision,open and reduction the fracture of femoral neck.Results:29 cases were followed up for 1～8 years ,27 cases were excellent,the rate of excellent and good was 93 percent.1 case had ischemic neurosis of femoral head,1 case had nonunion.Conclusions:The AO compression screws fixation and osteo periosteal bone grafts of deep iliac circumflex vessels is a good method to treat femoral neck fracture in young adults.It can make adequate blood to the necrotic femoral head,improve the rate of fracture union and reduce the complication.
