头颅磁共振及经颅多普勒超声在老年性痴呆早期诊断中的应用研究 被引量:2

摘要 目的评价头颅磁共振(MRI)及经颅多普勒超声(TCD)对老年性痴呆(AD)早期诊断的应用价值。方法对56例AD患者及年龄、性别相匹配的42例健康老年人进行MRI海巴结构体积测定及TCD脑血流动力学检测。结果头颅MRI显示,AD组较健康对照组海马萎缩程度明显,中重度AD组为(1.62±0.28)cm^3,轻度AD组为(2.10±0.44)cm^3,健康对照组为(3.12±0.18)cm^3(均为P<0.01)。TCD显示,AD组全脑平均血流速度明显减慢,且显著低于健康对照组(均为P<0.01),轻度AD组异常率为81.2%,中重度AD组异常率为89.1%。结论 MRI反映的海马体积萎缩及TCD显示的全脑平均血流速度减慢可辅助早期诊断AD,提高AD早期诊断的敏感性及准确性。 Objective To evaluate the worth that the magnatic resonance imaging (MRI) of head and ultrasonogra-phy doppler transcramal (TCD) are used to early diagnose of the Alzheimer disease (AD) . Methods To relativity 56 cases with AD and 42 cases healthy old people in age and sex, they are determined structure and volium of hippocampus by MRI and hemodynamic monitoring of bread. Results MRI showes that degree of hippocampus atrophy from AD group is evidentlier than healthy contrast group, hippocampus volium of the middle and severely group is 1.62 ± 0.28cm3. It of the light group is 2.10± 0.44cm3. It of the healthy group is 3.12± 0.18cm3 (P<0.01) . TCD shows that average hemodynamic speed of all bread is obviously slowlier and lower than healthy contrast group ( P < 0.01) . Abnormal rate of hemodynamic speed at the light group is 81.2%. It of the meddle and severely is 89.1 % . Conclusion MRI reports the degree of hippocampus atrophy and TCD showes average hemodynamic speed of all bread, they are plaied to diagnosis early AD. It is important to improve sensitiveness and accuracy of early diagnosis.
