原发性中枢神经系统淋巴瘤的诊断和治疗

Diagnosis Treatment of 65 cases with Primary central nervous system lymphoma (PCNSL)

目的探讨原发性中枢神经系统淋巴瘤(PCNSL)的诊断和治疗。对象国内文献59例、本院6例PCNSL。结果 PCNSL以神经系统表现为突出。CT、MRI等对诊断有重要帮助,但确诊靠病理检查。化疗应选用能通过血脑屏障的药物,放疗应足量。结论增强对PCNSL临床特征的认识是及时诊断该病的关键。PCNSL的治疗应化疗和放疗结合。

Objective To analyze the diagnosis and therapy for primary central nervous system lymphoma (PCNSL) .Method 59 cases from published data and 6 cases from our hospital were selected for analysis. Result The clinical features of PCNSL are predominated by its nervous manifestations. Diagnosis can be established only by pathological examination despite the absolute importance of CT and MRI. Chemotherapy should choose the drugs that have good permeability for blood brain barrier (BBB) .The dose of irradiation should be enough.Conclusion Understanding of its clinical characteristics is critical for the early diagnosis of PCNSL. The combination of chemotherapy and radiotherapy for PCNSL is recommended.
