烧伤残存示指移位再造拇指术

Thumb Reconstruction in Transposition of Residual Index Finger in Severely Burned Hand

目的对严重烧伤残缺手 ,通过改良的残存示指移位再造拇指 ,避免术中拇指血运障碍和术后肿胀。方法保留残存示指桡侧疤痕皮肤和周围组织 ,仅在示、中指指蹼间作纵形切口 ,分离截断示指掌骨 ,然后旋前位移向拇指。 结果 15例 16指残存示指移位再造拇指手 ,无一例发生术中血运障碍和术后肿胀。 结论在严重烧伤残存手采用该手术方法避免再造拇指的血运障碍和术后肿胀 ,手术操作简易。

Objective To suggest an improved method of thumb reconstruction by means of resiual index finger transposition in the severely burned hand. This method can prevent circulatory disturbance and swelling of the reconstructed thumb which takes place frequently under the regular method. Methods Keeping the radial-sided soft tissues intact, the residual index finger to be transposed was severed from the 2nd metacarpal bone of the middle finger by an incision over the web between the two fingers. Results Sixteen remained index fingers in 15 patients were transposed to the thumb position. No circulatory crisis or swelling of the reconstructed thumb swelling took place. Conclusion This is an easy and safe method for thumb reconstruction in the severely burned hand.
