万拉法新与丁螺环酮治疗伴有焦虑症状的抑郁症的疗效观察

Venlafaxine VS Buspirone in Treating Depression Associated with Anxiety.

目的 比较万拉法新与丁螺环酮治疗伴有焦虑症状的抑郁症的疗效。方法 随机将该类抑郁症病人分为万拉法新组 ( 31例 )与丁螺环酮组 ( 30例 ) ,两组均采用中等剂量治疗。以汉密而顿抑郁量表 ( HAMD)、汉密而顿焦虑量表 ( HAMA)观察 6周的疗效。并以副反应量表 ( TESS)观察不良反应。结果 6周后两组 HAMD减分值分别为 0 .6 8± 0 .1 5 ,0 .5 1± 1 .1( t=1 .37,P>0 .0 5 ) ,但第 2周末减分值前者显著高于后者 ( P<0 .0 5 )。HAMA减分值为 0 .77± 0 .1 2 ,0 .8± 1 .2 ( t=0 .2 9,P>0 .0 5 ) ,无显著差别。 TESS测 6周末增分率 ,万拉法新组为 37.2 1 % ,丁螺环酮组为 34 .32 % (χ2 =0 .2 4 ,P>0 .0 5 )无显著差异。结论 万拉法新抗抑郁效果优于丁螺环酮 ,而抗焦虑效果两者相仿 ,二者副反应均较小。

Objective To compare the efficacy of Venlafaxine and Buspirone in treating depression associated with anxiety.Method Patients of depression associated with anxiety(n=61)were randomly divided into 2 groups.Patients(n=31) in Venlafaxine group(A group)[M20 F11 age(31s9)18～42a]were treated with venlafaxine 125 mg/d Po for 6 weeks.The other 30 patients in Buspirone group(B group)[M 19 F11 age(32s18),19～43a]were treated with Buspirone 30 mg/d Po for 6 weeks.HAMD,HAMA and TESS were used to evaluate symptoms and clinical effects and side effects for 6 weeks.Results For patients completing 6 weeks of treatment with venlafaxine.The HAMD total score improved by 0.68±0.15 points.Improvement at 6 weeks was 0.57±1.1 points for patients treatedwith Buspironel(t=1.37,P>0.05).But at 2 weeks,the marked effective rate of venlafaxine group and buspirone group was 0.31±0.25 and 0.05±0.21 respectively(P<0.05)in treating depression symptoms.For patients completing 6 weeks,of treatment with venlafaxine,the HAMA total score improved by 0.77±0.12 points.Improvement at 6 weeks was 0.8±1.2 points for patients treated with buspirone(t=0.29,P>0.05).The increase of scores of TESS was 37.21%,34.32% in two group(χ 2=0.24,P>0.05).Conclusions Venlafaxine was superior in anti-depression efficacy to buspirone,Venlafaxine has the same anti-anxiety efficacy as buspirone,venlafaxine is effective and safe in treating depression associated with anxiety and it is superior to buspirone in reducing depression symptoms.
