门静脉主支癌栓PDGF-mRNA和E-CD蛋白的表达及意义

Expression of PDGF-mRNA and E-CD protein of primary hepatocellular carcinoma in portal vein tumor thrombosis and its significance

目的 探讨血小板源性生长因子 (PDGF mRNA)和内皮钙粘附蛋白 (E CD)在门静脉癌栓中表达水平的变化及其在肝癌肝内转移中的作用。方法 采用原位杂交技术检测 12例门静脉癌栓中PDGF mRNA基因的表达 ;采用免疫组化方法检测 33例门静脉癌栓中E CD蛋白和PDGF蛋白的表达 ,并与各自原发灶和癌旁组织进行对照。结果 (1)E CD蛋白的表达 :门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为 0 0 15 2± 0 0 0 36 ,0 0 5 2 7± 0 0 0 71和 0 10 0 8± 0 10 6 3。 (2 )PDGF蛋白的表达 :门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为 0 10 5 9± 0 0 10 4 ,0 0 5 2 1± 0 0 0 92和 0 0 10 8± 0 0 0 2 9。 (3)PDGF mRNA的表达 :门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为 0 12 0 7± 0 0 16 9,0 0 6 74± 0 0 0 71和 0 0 0 89± 0 0 0 0 6。经方差分析和t检验 ,各组间和各两组间均存在显著性差异 (P <0 0 1)。结论 门静脉癌栓中存在有促进转移和抑制转移两方面的因素 ,E CD蛋白表达减少 ,增加了门静脉癌栓中癌细胞脱落转移 ;而门静脉癌栓中PDGF mRNA表达的增加 ,则促进门静脉癌栓中结缔组织的形成 。

Objective To investigate changes in expression of PDGF mRNA and E CD protein of primary hepatocellular carcinoma in portal vein tumor thrombosis and explore its role in intrahepatic metastasis. Methods Expression of PDGF mRNA in 12 cases of portal vein tumor thrombosis of primary hepatocellular carcinoma was observed by in situ hybridization while that of E CD gene protein and PDGF protein in other 33 cases detected with immunohistochemistry. Meanwhile, their results were compared with primary tumor and paratumor tissue groups. Results The average light gaining index of expression of PDGF mRNA in portal vein tumor thrombosis group, primary tumor group and paratumor tissue group were 0 1207±0 0169, 0 0674±0 0071 and 0 0089±0 0006, respectively. The average light gaining index of expression of E CD gene protein in portal vein tumor thrombosis group, primary tumor group and paratumor tissue group were 0 0152±0 0036, 0 0527±0 0071 and 0 1008±0 1063, respectively. PDGF protein were 0 1059±0 0104, 0 0521±0 0092 and 0 0108±0 0029, respectively, in the 3 groups. There were significant differences among the 3 groups (P<0 01). Conclusions The expression of E CD protein decreased in portal vein tumor thrombosis increases the possibility of tumor ecchysis and metastasis from PVTT. The expression of PDGF mRNA and PDGF protein increased in portal vein promotes the formation of connective tissues that can curb tumor ecchysis and metastasis from PVTT.
