肾小球滤过率的相关因素分析及新测算公式的创建 被引量:12

摘要 目的 创建准确、简便、安全、价廉的测算肾小球滤过率 (GFR)新公式。方法 用99mTc DTPA清除率准确测定 12 2例受试者GFR(Tc GFR) ,同时测定血清肌酐 (SCr)等多种临床指标 ,经相关及回归分析 ,求出测算GFR(MC GFR)公式。以此公式测算另选的 83例受试者MC GFR ,并用Cockcroft/Gault公式测算其肌酐清除率(MC CCr)。结果 所求得的公式成立 ,其回归系数有显著性 (P <0 .0 0 0 1) ,lgMC GFR、lgMC CCr均与lgTc GFR有显著正相关 (P <0 .0 1~ 0 .0 5 ) ,但MC GFR与Tc GFR间无显著差异 (P >0 .0 5 ) ,而MC GFR与Tc GFR间则有显著差异 (P <0 .0 1)。结论 MC GFR可代替Tc GFR ,且准确、简便、安全、价廉 。 Purpose: To establish an accurate, simple, safe and cheap new formula for measurement and calculation of glomerular filtration rate (GFR). Methods: GFR from 122 cases of patient were determined accurately using Clearance of 99mTc-DTPA(Tc-GFR). Many clinical factors for example serum creatinine (SCr) and so on were determined simultaneously using routine method. The equation was obtained by the relationship and regression analysis for measurement and calculation GFR (MC-GFR). MC-GFR from another 83 cases of patient were measured and calculated by this equation and the creatinine clearance rate (CCr) were measured and calculated by Cockcroft/Gault formula (MC-CCr). Results: The equation was tenable and the regression coefficient was signif-icantly (P0.05) and there was difference significantly between MC-CCr and Tc-GFR (P<0.01). Conclusions: MC-GFR could replace Tc-GFR and MC-GFR was accurate, simple, safe and cheap and it should apply and popularize.
