胃术后残胃功能性排空障碍的诊治

Clinical study of the management for the patients suffering from gastric functional emptying disturbance after subtotal gastrectomy

目的 探讨胃术后残胃功能性排空障碍的病因及诊治。方法 对 10例胃术后残胃功能性排空障碍进行回顾性分析。结果 本组病例均经非手术疗法而治愈。结论 (1)发生该并发症主要原因是胃肠道运动的改变和失调、残胃及小肠尤其是空肠运动功能恢复缓慢及吻合口水肿 ;(2 )X线钡餐检查和胃镜检查 ,对确定诊断有重要意义 ;(3 )治疗应采取非手术疗法 ,包括禁食、胃肠减压、维持水电解质及酸碱平衡 ,应用胃肠动力药物 ,特别要注重加强营养支持治疗。

Objective To study the etiology,diagnosis and treatment for the patients suffering from gastric functional emptying disturbance after subtotal gastrectomy.Methods The etiology,diagnosis and treatment of 10 cases were analysed retrospectively.Results All patients in the group were cured by non surgical therapy.Conclusion (1)The complications result mainly from the change and disorder of gastrointestinal function,including the functional restoration of the movement of the residual stomach and small intestine makes slow progress,especially jejunum and anastomotic edema.(2)The complication can be exactly diagnosed by the examination of X ray of Barium and gastroscopy.(3)The non surgical therapy should be adopted,including fasting,keeping water,electrolyte and acid base balance,applying gastro dynamic drug and parenteral nutrition.
