腹腔镜处理慢性萎缩性胆囊炎508例体会

Laparoscopic Treatment for 508 Chronic Atropic Cholecystitis

目的 探讨腹腔镜处理慢性萎缩性胆囊炎的方法、技巧及注意问题。方法 回顾性分析 1 996年 5月～2 0 0 1年 8月腹腔镜处理的 50 8例慢性萎缩性胆囊炎临床资料。结果 496例行腹腔镜胆囊切除术 ,1 0例行腹腔镜中转开腹胆囊切除术 ,胆总管损伤 2例 ,行胆总管切开 ,T型管支撑引流术。胆漏 2例 ,经腹腔镜下置管引流保守治愈 ,全组均痊愈出院 ,无死亡病例。结论 术前明确诊断 ,术中规范性操作 ,术者及助手经验丰富 。

Objective\ To approach the technique and the points for attention in treating chronic atropic cholecystitis with the help of laparoscope. Methods\ A retrospective analysis was made of 508 cases of chronic atropic cholecystitis treated with laparoscope from May 1996 to August 2001.Results\ 496 cases underwent the excison of gallbladder by means of laparoscope and 10 cases converted to incision with 2 cases experiencing injury of choledoch which had to be incised with T tube for draining.A leak in choledoch happened to another 2 cases with pipe put in for draining. All the cases were healed without death cases. Conclusion\ Definite diagnose before operation,standard performance during operation with experienced performer and assistants are all important in treating chronic atopic cholecystitis with laparoscope.
