上气道测压阻塞定位系统在阻塞性睡眠呼吸暂停低通气综合征手术前评估的应用 被引量:2

摘要 目的通过分析上气道测压阻塞定位系统指导下的阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的手术疗效,了解上气道测压在OSAHS术前评估意义。方法对41例OSAHS患者进行上气道压力测定,根据上气道阻塞部位分为腭后区阻塞组和舌后区阻塞组,并采用对应的手术方式治疗。术后6~12个月复查多导睡眠检测(PSG),根据结果评估手术疗效。结果 41例患者中,治愈6例,显效25例,有效9例,无效1例,总手术成功率为75.61%;其中腭后区阻塞组27例,手术成功率为77.78%;舌后区阻塞组14例,手术成功率为71.43%。结论上气道压力测定能较准确地判断OSAHS患者的上气道阻塞平面,指导手术方案的制定,提高手术的疗效。 Objective To evaluate the surgical operation effectiveness under the guidance of upper airway pressure measurements of patients with obstructive sleep apnea-hypopnea syndrome ,understand the clinical significance of upper airway pressure measurements before OSAHS operation .Methods Forty-one OSAHS patients were divided into the retropalatal area and the retroglossal area blocking group ac -cording to the sites of upper airway pressure monitoring ,and operated with corresponding therapy .The PSG was reviewed 6 months to 1 year postoperatively,and evaluate the operation curative effect .Results Of the 41 cases,there were 6 cured cases,25 markedly effective cases,9 improved cases and 1 ineffective ease,and the total effective rate reached 75.61%.The operation success rate of 27 cases of retropalatal area obstruction was 77.78%,and the 14 cases of retroglossal area obstruction 71.43%.Conclusion The upper airway pressure measurement can accurately indicate the airway obstruction plane of OSAHS patients ,referring to electron nasopharyngolaryngoscopy ,provide guidance for operation scheme of OSAHS patients and improve the efficacy of operation .
