三维适形放疗联合CIK细胞治疗晚期原发性肝癌患者的疗效

Clinical observation of three-dimensional conformal radiotherapy combined with CIK for the treatment of advanced primary liver cancer

目的观察三维适形放疗(3D-CRT)联合细胞因子诱导的杀伤细胞(CIK)治疗晚期原发性肝癌患者的临床疗效。方法选取2013年8月至2015年8月中国人民解放军第105医院肿瘤中心就诊的72例晚期原发性肝癌患者,依据治疗方法不同分为对照组和观察组,每组36例患者。观察组在接受3D-CRT后3个月内给予CIK细胞治疗2次。治疗后3个月,依据肝脏CT、甲胎蛋白(AFP)、流式细胞术检测治疗前后免疫功能的变化情况,综合分析3D-CRT联合CIK细胞治疗晚期原发性肝癌的疗效。结果治疗结束3个月后,观察组的疾病控制率为80.6%,高于对照组的55.6%,差异有统计学意义(P<0.05);两组治疗后肿瘤直径和AFP指标监测数据显示,观察组[(4.17±1.65)cm和(348.5±127.2)ng/m L]明显优于对照组[(5.32±1.63)cm和(464.9±134.8)ng/m L],差异均有统计学意义(P<0.05);观察组CD+3T细胞、CD+4CD+8T抑制/细胞毒性细胞及CD+16CD+56NK细胞均不同程度增加(P<0.05)。结论在晚期原发性肝癌的治疗中,三维适形放疗联合CIK细胞治疗可以提高疾病的控制率、增强机体免疫力,有可行性,可进一步探讨。

Objective Observation of three dimensional conformal radiotherapy( 3D-CRT) combined with cytokine induced killer( CIK) for clinical efficacy of primary liver cancer patients in advanced stage. Methods In our hospital from August 2013 to August 2015 in cancer center for treatment of 72 cases of advanced primary liver cancer,patients were divided into control group and observation group,36 patients in each group. Observation group was treated with CIK for 2 times within 3 months after receiving 3D-CRT. 3 months after treatment,Comprehensive analysis of the efficacy of 3D-CRT combined with CIK for the treatment of advanced PLC,according to liver CT,alpha fetal protein( AFP),Karnofsky and the change of immune function of the patients treated by flow cytometry. Results After treatment,in the two groups of tumor diameter and AFP change in data,the observation group[( 4. 17 ± 1. 65) cm and( 348. 5 ± 127. 2) ng / m L]was significantly better than the control group[( 5. 32 ± 1. 63) cm and( 464. 9 ± 134. 8) ng / m L,P < 0. 05]. The surface markers of T cells and NK cells in the observation group were more than those of the control group( P < 0. 05). Conclusion In the treatment of advanced PLC,3D-CRT combined with CIK therapy can improve the disease control rate( DCR),enhance the immunity of the body,and it is feasible and can be further discussed.
