影响内瘘使用寿命的危险因素分析及防治措施

A study in risk factors for the duration of arteriovenous fistula in maintenance hemodialysis patients

目的分析影响内瘘使用寿命的危险因素 ,为制订有效的防治措施提供理论依据。方法对 6 8例内瘘闭塞患者进行研究 ,将内瘘使用寿命作为因变量 ,影响内瘘使用寿命的一系列因素作为自变量 ,用SAS软件包进行多因素回归分析。结果多因素回归方程总的P值为 0 0 0 0 1、R2 为 0 80 2 1、瘘平均使用寿命为 36 8± 19 4 (月 ) ,影响内瘘使用寿命的主要因素是 :年龄、每周透析次数、收缩压、甘油三脂、术后第一次使用瘘时间。结论影响内瘘使用寿命的因素很多 ,血透医务人员应积极采取防治措施 ,尽量延长内瘘寿命。

Objective To identify the risk factors for the duration of fistula in maintenance hemodialysis patients.Methods 68 maintenance Hemodialysis patients who suffered from arteriovenous fistula obstruction were recruited in this study and the relationship between their clinic characters and the arteriovenous fistula in the study was analyzed by SAS software.Results The duration of arteriovenous fisstula in the study was 36.83±19 42 months.There were significantly reverse relationships between the duration and age,dialysis times per week,systolic blood pressure,blood triglyceride level,time of the initial use of fistula after arteriovenous fistula operation.Conclusion There were many risk factors for the duration of A V fistula.Some cautions should be taken in patients with risk factors in order to prolong the duration.
