腹部外科手术出血量的观察和评估

Analysis of operative bleeding in the upper abdominal operations

目的调查腹部外科手术术中出血情况 ,比较各类手术出血量和不同计量方法的差异。方法采用 (1)敷料清点和吸引液计量法 ;(2 )术者经验估计法测定 110例上腹部手术患者的出血量。结果腹部外科手术的平均出血量 :胆囊切除术为 14 4ml,胃癌根治术为 4 6 7ml,肝切除术为 812ml,脾切和贲门周围血管离断术为 10 2 3ml。手术医师趋向于低估出血量 ,误差率为 7%～ 4 2 %。结论应充分估计术中出血量 ,做好准备提高手术质量。

objective To record and evaluate the volume of operative bleeding in upper abdominal operations.Methods A Prospective survey was made on 110 patients undergoing upper adominal Operations.The volume of bleeding was calculated by means of (1)record of blood in the suction and dressing,(2)estimation of operators.Results The volume of operative bleeding was 144ml in The cholecystectomy,467ml in subtotal gastroctomy and lymphonectomy,812ml in hepatectomy,1 023ml in esophagogastric devascularization.Conclusions The operators have underestimated the volume of operative bleeding for 7%～42%
