Anatomic basis on the blood supply of nasolabial fold random flap

目的 :为鼻唇沟皮瓣血供提供形态学基础。方法 :① 3 0例 (60侧 )经 10 %福尔马林固定的成人头部标本 ,动脉灌注红色乳胶 ,解剖观察鼻唇沟的动脉血供及静脉回流 ;②在 10侧头部铸型标本上 ,观测鼻唇沟皮肤和皮瓣蒂部的动脉血管分布及静脉回流 ;③ 1例新鲜成人尸体头颈部标本 ,从颈总动脉灌注明胶墨汁 ,切取鼻唇沟皮瓣并透明 ,观察血管分布及吻合。结果 :鼻唇沟部皮肤血供主要来源于面动脉、眶下动脉和面横动脉的分支 ,上述动脉发出小动脉入浅筋膜形成皮下动脉网 ,再由此发出分支至真皮下层形成真皮下动脉网。皮瓣蒂部血供来源于附近区域真皮下血管网。结论 :鼻唇沟皮肤血供极为丰富 ,任何一处均可为蒂形成真皮下血管网皮瓣 。

Objective:To provide anatomic basis for the blood supply of nasaolabial random flap.Methods:①The blood supply and venous return of nasolabial fold were observed on 30 adult specimens;②Vascular distribution of nasolabial fold skin and pedicle were observed on 10 sides of head cast specimens; ③The vascular distribution and anastomosis were observed on the transparent specimen of the nasolabial fold flap which perfused with gelatin Chinese ink.Results:The blood supply of nasolabial skin originated basically from the branches of the facial artery,infraorbital artery and transverse facial artery.The branches diverged from aforesaid arteries went to superficially and formed subcutaneous arterial network.The arteries from the network went to corium layer and formed subdermal arterial network.The pedicle was supplied by the subdermal vascular network of adjacent region.Conclusion:The blood supply of nasolabial skin is very rich.The survive of the nasolabial fold random flap depend basically on the subdermal vascular network.The main vessels are unnecessary to the pedicle.
