激素性股骨头坏死病程中股骨头局部VEGF mRNA表达的变化及意义

Change and Its Significance of the Expression of VEGF Messenger RNA of the Femoral Head in the Steroid-induced Avascular Necrosis of Femoral Head

目的 :探讨激素性股骨头坏死病程中股骨头局部血管内皮生长因子 (vascularendothelialgrowthfactor ,VEGF)mRNA表达的变化及意义。方法 :新西兰大白兔 3 6只 ,随机均分成正常对照组和模型组 ,每组 18只。模型组用内毒素加激素注射造成股骨头坏死。两组动物分别于造模后 6、10、14周各处死 6只行股骨头组织学检查和VEGFmR NA原位杂交观察。结果 :光镜观察示模型组股骨头骨小梁变细、空骨陷窝增加 ,成骨细胞数量减少 ,这些表现随着时间的推移逐渐加重 ;软骨下血窦计数减少 ,各时段均低于正常组 ,差异有显著性意义 (P <0 .0 5 ,或P <0 .0 1)。原位杂交显示 ,模型组兔的股骨头 6周时血管周围有VEGFmRNA阳性表达 ,10周时血管、成骨细胞的表达逐渐减少 ,14周时稍有所增加。结论 :股骨头坏死后血管修复能力下降 ,血管内皮细胞的功能受到破坏影响 ,增加股骨头局部VEGF可能有助于股骨头坏死的修复。

Objective: To explore the change and its significance of the expression of vascular endothelial growth factor(VEGF)messenger RNA of the femoral head in the steroid induced avascular necrosis of femoral head(ANFH).Method:36 New Zealand rabbits were randomly and equally divided into normal control group and model group.18 rabbits in model group were injected endotoxin and methylprednisolone.6 rabbits were killed at the time of the sixth week,tenth week,fourteenth week in each group respectively.Femoral head specimens were studied by histological examination and in situ hybridization.Results:By light microscopy,empty lacunae increased and numbers of osteoblasts and subchondral sinosoids decreased in model group.By in situ hybridization,expression of VEGF mRNA showed decrease in the osteoblasts and vascular endothelial cells in the femoral head of model group in the sixth week.Expression decased in the tenth week and increased slightly in the fourteenth week.Conclusior:The ability of blood vessel reparation dereases during avascular necrosis of the femoral head.Function of endothelial cells suffers the interruption as well.Increasing local VEGF in the femoral head may be beneficial to the reparative process of necrosis of the femoral head.
