高血压病患者心肌劳损与冠状动脉储备功能间的关系 被引量:1

The relationship between myocardial strain of hypertension and coronary flow reserve

摘要 目的 探讨高血压病患者心肌劳损与冠状动脉储备功能之间的关系。 方法 测定左冠状动脉主干基础状态下和静脉注射潘生丁后舒张期峰值血流速度 (PDV)、平均血流速度 (MDV)和舒张期血流速度时间积分 (DVI) ,计算用药后与用药前血流速度比值 (D/R) ,比较高血压病心肌劳损组与高血压病非心肌劳损组及正常组各测算值。结果 基础PDV、MDV和DVI值3组之间差异均无显著性意义 (均P>0.05) ,PDVD/R和MDVD/R值高压病心肌劳损组为2.02±0.45和1.91±0.36,显著低于正常组的2.92±0.58和3.0±0.82及高血压病非心肌劳损组的2.69±0.47和2.65±0.70(均P<0.01) ,高血压病非心肌劳损组与正常组比较差别无显著性意义 (P<0.05)。 结论 Objective To investigate the relationship between myocardial strain of hypertension and coronary flow reserve.Methods Coronary peak disatolic flow velocity(PDV),mean diastolic flow velocity(MDV)and diastolic flow velocity integral(DVI) of left main coronary were measured by multipe planes trans_esophageal echocardiogram before and after imtravenous injection of dipyridamole in group of hypertension with myocardial strain,group of hypertension without myocardial strain and normal group.The ratio of dipyridamole to rest velocity(D/R) was calculated.Results The baseline PDV,MDV and DVI were similar in 3 groups and the values of PDV D/R and MDV D/R were 2.02±0.45 and 1.91±0.36 in group of hypertension with myocardial strain,which were sinificant lower thant that in normal group(2.92±0.58 and 3.0±0.82) and that in group of hypertension without myocardial strain(2.69±0.47 and 2.65±0.70) (all P<0.01).There was no significant difference between latter two groups (P>0.05).Conclusion The electrocardiographic myocardial strain of hypertensive patients may indicate the reduced coronary flow reserve.
