成人创伤性与激素性股骨头缺血坏死全髋关节置换术后疗效分析

Total hip arthroplasty for treatment of osteonecrosis of the femoral head.——A comparison between trau-matic and steroid - induced osteonecrosis of the femoral head .

目的 比较研究创伤性与激素性股骨头缺血性坏死全髋关节置换术的疗效。方法 对我院1986年3月—1996年3月间行全髋关节置换术治疗的创伤性与激素性股骨头缺血性坏死患者进行随访研究。主要通过门诊复查、家访、信访等手段,收集96例患者(109髋)相关临床症状、体征及X线片等资料,并进行Harris髋关节功能评分及X线评价。结果 激素组松动率较创伤组高,两组松动率具有显著差异(P<0.05)。激素组翻修率3.6％,创伤组无一例翻修。结论 激素性股骨头缺血性坏死全髋关节置换术疗效较创伤性股骨头缺血性坏死差。

Objective To compare with the effect of total hip arthroplasty between the steroid - induced and the traumatic osteonecrosis of the femoral head. Methods It was followed up that 96 patients (109 hips) treated by total hip arthroplasties from March 1986 to March 1996 in our department. The follow- ups were mainly performed by consulting in OPD, home visits and communication inquiry. So the patients' clinical datas and X- ray films were collected and analyzed. All the patients were evaluated by Harris Hip Score (HHS) and X - ray films. Result The loosening rate of the steroid group was higher than the traumatic one. There was a significant difference of loosening rate between the two groups (P = 0.045 < 0.05) . The revision rate was 3.6% in the steroid group and none in the traumatic group. Conclusion The results of the steroid osteonecrosis treated by THA were inferior to the traumatic one.
