淀粉样脑血管病脑内老年斑及神经原纤维缠结的观察

摘要 连续尸解245例脑标本中,筛选出26例 CAA,以1:2配对形式选出52例脑标本作为对照组,观察脑内 SP 和 NFT。结果 CAA 组26例中20例(76.9%)见有 NFT,对照组52例中16例(30.8%)见有 NFT,差异非常显著。两组 NFT 分布均以海马较多。见有 SP 改变者,CAA 组8例(30.8%),对照组2例(3.8%),CAA组明显为多,差异有非常显著性。两组 SP 分布均以海马为多,而脑血管类淀粉物质沉积则以额、顶、枕、颞叶较多。本组根据 SP 及 NFT 的病理改变诊断为 Alzh(?)imer病者10例,其中 CAA 组9例,对照组1例,CAA 并存 AD 者明显为高。 26 cases of CAA were selected among 246 caes ofconsecutive autopsies.and 52 specimens of brains were chosenwith the ratio one to two as control to observe the SP and NFTin brains.In result.NFT was found in 20 cases(76.9%)among 26 cases of CAA.and 16 cases(30.8%)among 52controls in the group.The difference is very significant.NFTmostly distributed in hippocampus in both groups.8 cases of SPwere found in the CAA group and two cases(3.8%)in thecontrol group.The significant difference were markedly higherin the CAA group.Distribution of SP in both group is mainly inthe hippocampus.while the sedimentation of cerebrovascularamyloid is mainly in the frontal.parietal temporal and occipitallobes.10 cases were diagnosed as Al:zheimer's diseaseaccording to the pathological changes of SP and NFT in thispaper in which 9 cases were in CAA group and one in control.Itis obviously higher in CAA associated with AD.
