老年护理院与家庭病床双向转诊评分标准模式的拟定与评价

Formulating and evaluating the standard of giving marks for two-way diagnosis and treatment between elderly nursing-hospital and family_bed

目的 拟定并评价老年护理院与家庭病床双向转诊的评分标准。方法 综合病人日常生活活动能力及病情确立评分标准 ,随机选择 2 0 0 0年符合评分标准的老年护理院病人 1 0 0例 ,家庭病床病人 1 0 0例为A1 、B1 组 ,进行评分、双向转诊和有关费用调查 ,并与对照组 2 0 0 0年不符合评分标准的老年护理院病人 1 0 0例、家庭病床病人 1 0 0例的A2 、B2 组进行比较。结果 A1 组药品费、护理费占住院总费百分比 ,B1 组药品费占家床总费百分比与对照组间差别有显著性意义 (P <0 0 5) ,B1 组查房诊疗护理费占家床总费用百分比与对照组间差别无显著性意义 (P >0 0 5)。结论 所拟老年护理院与家庭病床双向转诊评分标准的模式 ,能够降低医疗费用 ,节约卫生资源 ,为一级医院功能定位在社区提供了保证。

Objective To formulate and evaluate the standard of giving marks for two_way diagnosis and treatment between elderly nursing_hospital and family_bed.Methods To analyze patients activities in their daily life and conditions of their illness to draw up the standard of giving marks.Accord with standard′s 100 patients who were in the elderly nursing_hospital in 2000,and 100 patients who were in the family_bed were chosen randomly to form two groups A 1?B 1.There two groups were given marks,two_way diagnosis and treatment,and their expenses were investigated and then we contrasted group A 1 and group B 1 with group A 2,in which there were not accord with standard′s 100 patients who were in the elderly nursing_hospital and group B 2,where there were 100 patients in the family_bed in 2000.Results Compared with control group,group A 1,average hospitalization_days;the ratio of medicine expenses to hospitalization expenses to the whole hospitalization expenses;group B 1,the ratio of the drug expenses to all the family_bed expenses,there is notable difference(P<0 05 or P<0 01),but group B1,the ratio of the nursing expenses to all the family_bed expenses,there is no obvious difference (P>0 05).Conclusion This model,the standard of giving marks for two_way diagnosis and treatment between Elderly Nursing_hospital and Family_bed,can reduce medical expenses,raise the rate of the utilization of beds,save health resource and ensure grade Ⅰ hospital to function in community.
