静脉全麻药异丙酚与硫喷妥钠对脑代谢影响的比较 被引量:1

摘要 目的 比较静脉全麻药异丙酚、硫喷妥钠对脑代谢的影响。方法 选择头颈和上腹部全麻病人 40例 ,ASAⅠ~Ⅱ级 ,年龄 2 0~ 60岁。随机分为 2组 :硫喷妥钠组 (S组 ) 2 0例 ,用硫喷妥钠 (5mg kg) +芬太尼 +司可林诱导 ;异丙酚组 (P组 ) 2 0例 ,用异丙酚 (2 .5mg kg) +芬太尼 +司可林诱导。检测全麻诱导前 (T0 )和插管后 2min(T1 )、5min(T2 )、1 5min(T3 )各时点的乳酸浓度和动脉血和颈内静脉血血气分析。结果  2组动静脉乳酸浓度差(DLa-jv)诱导后逐渐下降 ,T2 、T3 较T0 下降明显 (P <0 .0 5) ,组间无差异 ;2组颈内静脉氧饱和度 (SjvO2 )诱导后均增高 ,T1 时至峰值 ,P组、S组分别增加 2 9.6 %、2 1 .3 % ,然后逐渐回落 ,组间无差异 ;2组脑氧摄取率 (ERO2 )、动静脉氧含量差 (Da-jvO2 )诱导后开始下降 ,T1 时降至最低 ,组间无差异。结论 静脉全麻药异丙酚与硫喷妥钠均可降低脑代谢 ;作用程度上异丙酚较硫喷妥钠有增强的趋势 ,但无统计学意义。
