曲普瑞林在试管内受精中的降调节作用及对促性腺激素用量的影响

Triptorelin′s down-regulation in in vitro fertilization and its influence on the gonadotrophin dosage

目的 探讨应用曲普瑞林 (triptorelin)在试管内受精 (IVF)中的降调节作用及其与使用促性腺激素用量的关系。方法 通过对接受 IVF治疗的 6 0例不孕症妇女随机分组 ,A组 (triptorelin组 ) 30例 ,B组 (布舍瑞林 ,buserelin组 ) 30例 ,观察应用 Gn RH兴奋剂后阴道出血第 3天血卵泡刺激素 (FSH)及雌二醇 (E2 )值及促卵泡激素的用量。结果 A组的血 FSH及 E2 值明显低于 B组 ,差异有显著性 (P<0 .0 1)。 A组的促性腺激素用量明显多于 B组 ,差异有非常显著性 (P<0 .0 1)。结论 应用曲普瑞林后 10～ 14d,垂体极度抑制 ,此时的卵巢也处于极低的功能状态 ,从而增加了 Gn征集卵泡及促卵泡发育的用量。

Objective To study the down-regulation and the effect of triptorelin on the total dosage of gonadotrophins in in vitro fertilization (IVF).Methods Sixty infertile women wanting to receive IVF were randomly divided into two groups:group A (n=30) triptorelin given,group B (n=30) buserelin given,observing FSH and E 2 levels of D 3 after using GnRH-a and the total dosage of gonadotrophins.Results FSH and E 2 levels were statistically lower in group A than in group B (P<0.01).The total dosage of gonal-F of group A was statistically larger than group B (P<0.01).Conclusion The pituitary is suppressed while the function of ovaries is poor after using triptorelin,so it increases the total dosage of gonadotrophins collecting and developing follicles.
