肝动脉化疗栓塞对肿瘤血管生成影响的实验研究 被引量:18

Effect of transarterial chemoembolization on angiogenesis of rat walker-256 transplanted hepatoma

摘要 目的 研究化疗栓塞术后残存肿瘤组织中微血管密度 (microvesseldensity ,MVD)、血管内皮生长因子 (vascularendothelialgrowthfactor,VEGF)及碱性成纤维细胞生长因子 (basicfibroblastgrowthfactor,bFGF)的表达情况及其意义。方法  6 0只大鼠Walker 2 5 6肝癌模型用简单随机抽样法随机分为 3组 ,每组 2 0只。对照组 :经肝动脉灌注生理盐水 0 2~ 0 3ml;单纯化疗组 :灌注 5 氟尿嘧啶(5 Fu) 2 0mg/kg ;化疗栓塞组 :灌注超液态碘油 0 5ml/kg及 5 Fu 2 0mg/kg。术后 2周用免疫组织化学(简称免疫组化 )方法检测各组von Willebrandfactor(Ⅷ因子 )、VEGF及bFGF的表达情况 ,对Ⅷ因子阳性血管内皮进行MVD计数。并结合其病理分布特征进行分析。结果 对照组、单纯化疗组及化疗栓塞组MVD值分别为 80 84± 2 4 2 4、83 0 5± 2 0 2 9及 85 2 0± 2 3 91(血管个数 /视野 ) ,VEGF阳性率分别为 75 %、75 %及 85 % ,bFGF阳性率分别为 30 %、2 5 %及 30 % ,各组间均无统计学差异 (F =0 193,χ2 =0 4 4 9,0 14 1,P值均 >0 0 5 )。MVD与VEGF之间存在较好的相关性 (r =0 4 5 2 ,F =2 3 2 92 ,P <0 0 1)。结论 化疗 碘油栓塞对肿瘤新生血管的形成影响较小。新生血管的形成及VEGF的表达对栓塞后侧支循环? Objective To investigate intratumor microvessel density (MVD), expression of VEGF and bFGF in survival cancerous tissue after chemoembolization and evaluate their significance Methods Sixty rats bearing walker 256 transplanted hepatoma were randomly divided into control group, chemotherapy group and chemoemoblization group Each group included 20 rats Saline, 5 Fu, 5 Fu and lipiodol were infused through hepatic artery, respectively Two weeks after the infusion, stain of factor Ⅷ, VEGF and bFGF were performed by the immunohistochemical method in routine paraffin embeded section MVD was counted by endothelial cells which were highlighted by factor Ⅷ Their expression levels were analyzed in conjunction with the pathological feature Results MVD of control group, chemotherapy group and chemoemoblization group was 80 84±24 24, 83 05±20 29 and 85 20±23 91, respectively The positive express of VEGF and bFGF was 75%, 75%, 85% and 30%, 25%, 30 %, respectively Statistical analysis revealed a positive correlation between the express of VEGF and MVD Conclusion Embolization by lipiodol cannot thoroughly eliminate the angiogenesis of the tumor Formation of neovascularity and express of VEGF may play an important role in re establishment of blood supply of the survival tumor tissues after chemoembolization
