云南省华宁县769例健康人群乙型肝炎疫苗免疫水平

Surveillance on immunity level of hepatitis B vaccine among 769 healthy people in Huaning County of Yunnan

目的掌握云南省华宁县健康人群乙型肝炎(乙肝)病毒感染状况及免疫水平,评价乙肝疫苗接种及纳入计划免疫的效果,为提高我县免疫规划工作提供科学依据。方法根据《全国人群乙肝等有关疾病血清学调查方案》的要求,结合华宁县具体情况,按容量比例概率抽样法(PPS)随机抽取769例健康人群,采用酶联免疫吸附试验(ELISA)检测乙肝表面抗原及抗体。结果调查的769例健康人群中乙肝疫苗接种率为77.11%,乙肝表面抗原阳性率为0.39%、乙肝表面抗体阳性率为75.81%,乙肝疫苗接种保护率为94.56%。不同乡镇、不同年龄组间乙肝疫苗接种率、乙肝表面抗体阳性率差异有统计学意义;性别间差异无统计学意义。结论华宁县健康人群乙肝疫苗接种保护率较高,乙肝疫苗免疫效果显著,还要加强重点人群的乙肝疫苗接种工作,巩固乙肝免疫效果。

[Objective]To understand the infection status and immunity level of hepatitis B among healthy population in Huaning County of Yunnan,evaluate the effect of hepatitis B vaccine and planned immunity,and provide scientific basis for improving EPI in Huaning County.[Methods]According to the National serological survey protocol for hepatitis B and related diseases,combined with the specific circumstance of Huaning County,769 healthy people were randomly sampled by the probability proportional to size sampling（ PPS）. Enzyme linked immunosor bent assay（ ELISA） was used to detect HBsAg and HBsAb. [Results]Among 769healthy people,the immunization coverage rate of hepatitis B vaccine was 77. 11%,the positive rate of HBsAg and was HBsAb was0. 39% and 75. 81% respectively,and the protection rate of hepatitis B vaccine was 94. 56%. The differences in the immunization coverage rate of hepatitis B vaccine and the positive rate of HBsAb between different townships and different age groups were statistically significant,while there was not significant difference between males and females.[Conclusion]The protection rate of hepatitis B vaccine is high among healthy population in Huaning County,and the immunization effect is significant. However,it is necessary to strengthen the hepatitis B vaccination in high risk population,to consolidate the immune effect.
