2型糖尿病患者胰岛素抵抗与中医辨证分型的相关性研究

Study on Relationship Between Insulin Resistance and TCM Syndrome Differentiation of Type 2 Diabetic

目的:对2型糖尿病(DM)患者的胰岛素抵抗(IR)与中医辨证分型的相关性进行分析,为临床辨证诊治提供相关科学依据。方法:将200例2型DM患者作为观察组,选取同期100例健康体检者作对照组,检测前3 d停用降糖药物,抽取空腹静脉血液4 mL,分析两组中医辨证分型的IR与血胰岛素(FINS)、空腹血糖(FBG)、生长抑素(SS)、胰岛素敏感指数(IAI)与病程的相关性。结果:①IR与FINS、FBG指标具正相关性(P<0.05),但IR与SS、IAI与病程具有负相关性(P<0.05);②观察组中医辨证分型血糖、血胰岛素、IAI低于对照组(P<0.05);且观察组各分证间指标存在明显差异(P<0.05);③观察组中医辨证分型SS与IR指标与对照组差异显著(P<0.05);且观察组各分证间指标存在明显差异(P<0.05)。结论:通过分析2型DM患者IR客观指标与中医辨证分型间的关系,为中医降糖治疗提供有效的临床依据。

Objective： To investigate relationship between insulin resistance and TCM syndrome differentiation of type 2 diabetic to provide scientific basis for clinical dialectical treatment. Methods： The 200 cases of type 2 DM cases,and 100 cases of normal population health examination were selected,the former was divided into the observation group and the latter was divided into the control group. In the detection of 3 day discontinuation of hypoglycemic drugs,fasting venous blood from 4 mL. Determine and analyze the correlation between TCM syndrome types with IR,trypsin,FBG,SS,IAI and pathogenesis. Results： ①IR and FINS,FBG indicators with positive correlation（P 0. 05）,but the IR and SS,IAI and the course has a negative correlation（P 0. 05）; ②TCM type of blood glucose,plasma insulin and IAI sensitivity index of observation group were lower in the control group（P 0. 05）. There were significantly different（P 0. 05） between the points card index of observation group. ③TCM syndrome type SS index and HOMA- IR had significant difference in observation group（P 0. 05）. Each syndrome type index in observation group had significantly difference（P 0. 05）. Conclusion：Through the analysis of relationship between insulin resistance and TCM syndrome differentiation of type 2 diabetic so as to provide clinical basis for TCM therapy.
