支气管肺泡灌洗液中粒细胞酯酶水平与支气管炎性疾病的相关性研究

Correlation between neutrophils esterase levels in bronchoalveolar lavage fluid and bronchial diseases

目的探讨支气管肺泡灌洗液中粒细胞酯酶与支气管炎性疾病的相关性。方法对2008年1月至2012年12月458例肺部炎性疾病门诊患者进行支气管镜检,评价气道炎症程度,收集支气管肺泡灌洗液做细胞分类计数,测定灌洗液中粒细胞酯酶水平,分析粒细胞酯酶水平与疾病炎症程度的相关性。结果支气管肺泡灌洗液中粒细胞酯酶水平与支气管镜下气道炎症程度、C反应蛋白、灌洗液中粒细胞比率相关。粒细胞酯酶在急性气道感染性疾病患者明显升高,在慢性炎性疾病及非感染性疾病无明显增高。结论监测支气管肺泡灌洗液中粒细胞酯酶水平有助于评估肺部炎性疾病的炎症程度,对鉴别急慢性炎症性疾病及非感染性疾病有一定价值。

Objective To explore the correlation between neutrophil esterase in bronchoalveolar lavage fluid and bronchial diseases.Methods A total of 458 patients with lung inflammatory diseases from January,2008 to December,2012 were included.Bronchoscopy was applied to evaluate the degree of airway inflammation.Bronchoalveolar lavage fluid was collected to perform classified counting of leucocyte and measure neutrophils esterase levels.Its clinical significance was estimated by combination with clinical information.Results Neutrophils esterase levels in bronchoalveolar lavage fluid were correlated with bronchoscopic airway inflammation and serum inflammatory markers as well as the lavage fluid neutrophil ratio.Neutrophil esterase was significantly elevated in patients with acute airway infectious diseases while it was not significantly increased in chronic inflammatory diseases and non-infectious diseases.Conclusion Neutrophils esterase levels in bronchoalveolar lavage fluid are related to bronchoscopic airway inflammation grading and inflammatory cell ratio as well as serum inflammatory markers.The detection may help to assess the degree of inflammation and differentiation diagnosis between acute inflammatory disease and chronic inflammatory diseases or non-infectious diseases.
