经阴道彩色多普勒超声对剖宫产瘢痕妊娠的诊断价值

Diagnostic value of TVCDS in the cesarean scar pregnancy

目的探讨经阴道彩色多普勒超声(transvaginal color doppler scan,TVCDS)对剖宫产术后子宫瘢痕妊娠(cesarean scar pregnancy,CSP)的临床诊断价值。方法选取2013年2月至2013年11月四川省人民医院妇科收治的临床初诊为CSP患者45名,对其进行TVCDS检查,追踪其病理诊断结果,并对CSP的声像图表现及血流频谱进行回顾分析。结果 38例CSP患者的经TVCDS诊断与最终病理诊断结果一致,诊断符合率93.3%。TVCDS对CSP诊断灵敏度为95.0%,特异度为80.0%,阳性预测值97.4%,阴性预测值66.7%,诊断符合率93.3%。结论 TVCDS对CSP具有较高的诊断符合率,可作为CSP的首选诊断方法。

Objective To evaluate the diagnostic value of transvaginal color doppler scan（TVCDS） in the cesarean scar pregnancy（CSP）.Methods Forty-five patients who were first diagnosed as uterine scar pregnancy at the department of gynecology of our hospital from February 2013 to November 2013 were selected.They were examined with TVCDS.Their pathological diagnosis was tracked.The ultrasonographic manifestations and blood flow spectrum of the patients were retrospectively analyzed.Results There were38 cases whose TVCDS diagnosis was consistent with the final pathologic diagnosis（diagnostic coincidence rate was 93.3%）.The sensitivity,specificity,positive predict value and negative predict value of TVCDS for CSP diagnosis were 95.0%,80.0%,97.4% and66.7%,respectively.Conclusion TVCDS is accurate for CSP diagnosis.It can be the first-choice method in the examination of the disease.
