摘要
目的通过对5·12汶川地震伤员外周神经损伤的高频超声检查,探讨地震伤员外周神经瘤的超声特征。方法对我院骨科、地震康复中心和四川省绵竹市板房医院的具有周围神经损伤症状的5.12汶川大地震伤员进行高频超声检查,并与手术结果比较。结果检查出创伤性神经瘤19个,瘤体最小的约0.8 cm×0.4 cm,最大的约2 cm×3 cm。其中9个发生在截肢伤员的残肢神经末端,2个发生在神经完全离断的断端,2个为神经部分断裂后形成,2个为瘢痕卡压神经所致,4个发生在神经断裂后手术连接的吻合口。结论高频超声检查能对地震所致的创伤性神经瘤准确定位,并可清楚地显示创伤性神经瘤的大小、形态、与周围组织的毗邻关系,对评价手术后康复疗效和选择再次手术的方式具有重要的意义。
Objective To investigate the ultrasonographic characteristics of traumatic neuromas caused in earthquake by using high-frequency ultrasound to examine the injured peripheral nerve of casualties in the 5.12 Wenchuan earthquake.Methods Casualties of the 5.12 Wenchuan earthquake,who had abnormal symptoms in the peripheral nerve,were examined with high-frequency ultrasound in the Earthquake Rehabilitation Center,Sichuan Provincial People's Hospital and Mianzhu Temporary Hospital.The results were compared with that of operation.Results Nineteen traumatic neuromas were detected.Of these,the smallest one was about 0.8×0.4cm while the biggest one was about 2×3 cm.Nine cases occurred in the nerve terminal of amputees,2 cases occurred in the end of complete ruptured nerve,2 cases were caused by nerve partial rupture,2 cases formed because scar pressed nerve and 4 cases were found at surgical anastomosis.Conclusion High-frequency ultrasonography can not only locate traumatic neuromas correctly.But also is able to display tumor's shape and relationship with the surrounding structures.Therefore,it has important significance to evaluate effects of postoperative rehabilitation and choose better surgical modes.
