摘要
目的分析凶险型前置胎盘的临床特点及围手术期处理,以降低严重产后出血和子宫切除风险。方法回顾分析2008年1月至2013年1月我院收治的凶险型前置胎盘86例(研究组)的临床特点,并与同期普通型前置胎盘277例(对照组)比较。结果研究组的术中出血量、产后24小时总出血量、新生儿重度窒息、新生儿死亡率、胎盘植入发生率、弥漫性血管内凝血(DIC)发生率及子宫切除率均显著高于对照组,差异有统计学意义(P<0.05)。结论凶险型前置胎盘因瘢痕部位失去有效收缩能力及胎盘植入率高,更易发生严重的产时、产后出血,DIC及子宫切除风险明显增加,严格掌握剖宫产指征,重视前置胎盘围手术期处理及产后出血抢救环节是治疗成功的关键。
Objective To explore the clinical characteristics and perioperative management of pernicious placenta previa in order to decrease the risks of serious postpartum hemorrhage and hysterectomy.Methods The clinical characteristics of 86 cases with pernicious placenta previa were retrospectively analyzed and compared with 277 cases with general placenta previa.Results There were significant differences in the hemorrhage volume of intrapartum and postpartum,rate of placenta implantation,DIC,newborn severe asphyxia,newborn death and hysterectomy between the two groups（P 〈 0.05）.Conclusion The pernicious placenta previa,due to the loss effective contractility at scar sites,has a high rate of placenta implantation that is susceptible to serious intrapartum and postpartum hemorrhage.To control the indicators of caesarean,pay attention to perioperation period treatment of placenta previa and take the appropriate management of postpartum hemorrhage are the key for successful treatment.
