尖锐湿疣在肛门及肛管病变中人乳头状瘤病毒感染的基因分析

Genotyping of human papillomavirus causing infections in patients with condyloma acuminate in anus and anal canal

目的探讨肛门及肛管尖锐湿疣病变中人乳头状瘤病毒(HPV)感染的基因型,并对其临床意义进行分析,以降低感染率。方法收集2010年1月-2012年12月的肛门、肛管石蜡组织标本240例,对其组织的HPV基因型进行检测分析;数据采用SPSS11.0统计软件进行分析。结果在240例肛门及肛管尖锐湿疣患者中HPV基因检测阳性171例,HPV感染率为71.25%,其中一重感染130例占76.02%,在一重感染中,HPV 6型是检出率最高有62例阳性检出率为25.83%;其次是HPV 11型有59例阳性检出率为24.58%;多重感染的患者有41例占23.98%,而在多重感染中HPV 6+11型占的比例最大有21例占51.22%;检测的23种基因型中共检测出基因型20种,未发现HPV 18、35、56型,主要的HPV阳性基因型的分布及检出率男性为78.43%、女性为82.73%,两者差异无统计学意义,而HPV 6型的感染率男性为50.98%、女性为40.00%,两者差异有统计学意义。结论 HPV 6型、HPV 6+11型是肛门以及肛管尖锐湿疣的主要致病类型。

OBJECTIVE To explore the genotypes of human papillomavirus （HPV） causing infections in patients with condyloma acuminate in anus and anal canal and analyze the clinical significance so as to reduce the infection rate. METHODS Totally 240 paraffin tissue samples of anus and anal canal were collected from Jan 2010 to Dec 2012, then the genotypes of HPV were detected and analyzed, and the statistical analysis of data was performed with the use of SPSSll. 0 software. RESULTS Of the 240 patients with condyloma acuminate in the anus or anal canal, 171 cases were tested positive for the genes of HPV with the infection rate of 71. 25%, including 130 （76.02%） cases of single infection and 41 （23.98%） cases of multiple infections. Among the patients with single infection, there were 62 （25.83%） cases of HPV 6 infection and 59 （24.58%） cases of HPV 11 infection. Among the patients with multiple infections, there were 21 （51.22%） cases of HPV 6 plus 11 infection. Totally 20 of 23 genotypes have been detected, but the HPV 18, 35, or 56 genotype has not been detected. The positive rate of the major HPV genotype was 78.43% in the male patients, 82.73% in the female patients, and the difference was not significant; however, the incidence of HPV 6 infection was 50.98% in the male patients, 40.00% in the female patients, and the difference was significant. CONCLUSION The HPV 6 and HPV 6 plus 11 are the predominant genotypes of HPV causing condyloma acuminate in anus and anal canal.
