摘要
目的探讨性激素对绝经后女性冠心病发病的影响。方法选择2012年3月—2013年3月在我院心内科就诊的绝经1年以上女性139例,根据冠状动脉造影结果将其分为冠心病组67例和无冠心病组72例;另选择同期在我院体检健康育龄期女性63例为对照组。检测3组女性的性激素水平、血脂谱及血清内皮素-1、同型半胱氨酸(HCY)、纤维蛋白原(FIB)水平;相关性分析采用Pearson直线相关性分析。结果 3组女性血清睾酮水平比较,差异无统计学意义(P>0.05);冠心病组女性血清雌二醇、HDL-C水平低于对照组和无冠心病组,血清TC、TG、LDL-C、HCY、内皮素-1、FIB水平高于对照组和无冠心病组,血清孕酮水平低于对照组(P<0.05)。Pearson直线相关性分析结果显示,血清LDL-C(r=-0.43,P=0.000)、FIB(r=-0.38,P=0.001)、内皮素-1(r=-0.30,P=0.015)、HCY(r=-0.41,P=0.000)水平均与血清雌二醇水平呈负相关。结论绝经后女性性激素变化尤其是血清雌二醇水平下降与冠心病发病密切相关,其作用机制可能与血管内皮功能、脂代谢及纤溶系统功能异常有关。
Objective To investigate the influence of sex hormone on onset of coronary heart disease（CHD）in post-menopausal women. Methods From March 2012 to March 2013,67 postmenopausal women with CHD（ CHD group）,72 postmenopausal women without CHD（non - CHD group）and 63 premenopausal women without CHD（control group）were se-lected,and the serum levels of sex hormone,blood lipid,ET - 1,homocysteine（HCY）and fibrinogen detected. Results There was no significant differences of serum testosterone levels in the three groups. The serum levels of estradiol,HDL - C in CHD group was significantly higher than those in the other two groups（P 〈 0. 05）,while the serum levels of TC,TG,LDL -C,HCY,ET - 1,FIB were significantly higher than those in the other two groups（ P 〈 0. 05）,serum progestin level lower than in control group. By Pearson linear correlation analysis,serum levels of LDL - C（r = - 0. 43,P = 0. 000）,FIB（r = -0. 38,P = 0. 001）,ET - 1（r = - 0. 30,P = 0. 015）,HCY（r = - 0. 41,P = 0. 000） were all negatively correlated with serum estradiol level. Conclusion The changes of sex hormone were correlated with onset of CHD in postmenopausal women, especially the decrease of serum estradiol level,the mechanism maybe correlated with damage of endothelial function,lipid me-tabolism and fibrinolysis system.
