显微镜下外侧裂入路手术治疗高血压壳核出血的临床疗效评价

Clinical Effect of Lateral Fissure Approach Operation under Microscope on Hypertensive Putaminal Hemorrhage

目的评价显微镜下外侧裂入路手术治疗高血压壳核出血的临床疗效。方法选择本院2010年5月—2012年5月收治的高血压壳核出血患者87例,将患者随机分为外侧裂组49例和对照组38例。外侧裂组进行显微镜下外侧裂入路手术,对照组进行骨瓣开颅颞叶手术,比较两组临床效果。结果术后随访6个月～1年,外侧裂组患者术后恢复效果优于对照组（P〈0.05）。外侧裂组术后并发症发生率为10.2%（5/49）,低于对照组的28.9%（11/48）（P〈0.05）。结论显微镜下外侧裂入路手术治疗高血压壳核出血术后恢复效果好,且并发症少。

Objective To evaluate the clinical effect of lateral fissure approach operation under microscope on hyper-tensive putaminal hemorrhage. Methods 87 patients with hypertensive putaminal hemorrhage in this hospital were chosen from May 2010 to May 2012,and they were randomly divided into the lateral fissure group of 49 cases and the control group of 38 ca-ses. The lateral fissure group was performed lateral fissure approach operation under microscope,the control group was performed craniotomy temporal lobe operation,the clinical effect were compared. Results The follow - up lasted for 6 months to 1 year, the postoperative recovery effect of lateral fissure group was better than that of control group（P 〈 0. 05）. The postoperative com-plication rate of lateral fissure group was 10. 2%（5 / 49）,was lower than that of control group of 28. 9% （11 / 48）（P 〈 0. 05）. Conclusion Lateral fissure approach operation under microscope has a better postoperative recovery effect on hypertensive pu-taminal hemorrhage,and has fewer complications.
