外周型原始神经外胚层肿瘤的X线、CT和MRI影像分析

摘要 目的分析外周型原始神经外胚层肿瘤(pPNET)患者X线、CT、MRI影像学检查方式及特点。方法回顾性分析10例经病理证实的PPNET患者X线、CT、MRI影像学检查资料,并结合病理检查结果分析病变部位影像学特征。结果发病部位为骨骼6例(包括左胫骨1例、右胫骨2例、骶骨1例、髂骨1例、左肱骨1例),软组织4例(纵隔2例、胸壁1例、腹膜后1例)。X线检查发现以溶骨性骨质破坏为主;CT检查主要表现为不规则溶骨性骨质破坏、边界模糊的不规则软组织肿块影,肿块内多有囊变、坏死;MRI表现主要为T1WI为稍低或中等信号、T2WI呈不均匀高信号。结论 X线、CT及MRI检查均可较好地观察病灶内部结构及周边变化,合理联用可达到理想的定性、定量诊断效果,还可指导手术切除、远处转移分析及疗效评价。 Objective Analysis of peripheral primitive neuroectodermal tumors (PPNET) examination and X-ray features in patients with CT, MRI, image. Methods A retrospective analysis of 10 cases of pathologically confirmed PPNET patients with X-ray, CT, MRI imaging data, combined with pathological examination results analysis of the imaging characteristics of lesions were. Results 6 cases of skeletal sites (including the left tibia in 1 cases, right tibia in 2 cases, 1 cases, 1 cases of sacrum ilium, left humerus in 1 cases), 4 cases of soft tissue (2 cases, mediastinal chest wall in 1 cases, retroperitoneal in 1 cases). X-ray examination found in osteolytic bone destruction;CT examination mainly showed irregular osteolytic bone destruction, fuzzy boundaries and irregular soft tissue mass, multiple cystic necrosis, tumor;MRI for the performance of T1WI is slightly low or medium signal, T2WI showed inhomogeneous high signal. Conclusion X-ray, CT and MRI examination can observe the internal structure and the surrounding lesions change better, with the appropriate combination of qualitative, quantitative diagnosis can reach ideal effect, still can guide the operation excision, the evaluation of distant metastasis and efficacy analysis.
