硬膜外麻醉导管行脐静脉置管在新生儿复苏中的应用研究

Epidural Anesthesia Catheter in Umbilical Vein Catheter Applied Research in Neonatal Resuscitation

目的探讨硬膜外麻醉导管行脐静脉置管在新生儿复苏中的应用效果。方法选取2008年1月至2010年12月在我院出生的、进行脐静脉窒息复苏的新生儿36例为对照组,2011年1月至2014年4月在我院出生的、进行脐静脉窒息复苏的新生儿36例为观察组。对照组采用传统的脐静脉注射法。观察组采用硬膜外麻醉导管行脐静脉置管。观察一次性置管成功率,注射顺畅度,药液外渗率,脱出情况等指标。结果观察组硬膜外麻醉导管行脐静脉置管一次成功率100%,对照组穿刺一次成功率为66.66%(24/36)。两组比较,观察组显著优于对照组,差异具有统计学意义(P<0.05)。两组患儿并发症总发生率比较,观察组显著低于对照组,差异具有统计学意义(P<0.05)。结论硬膜外麻醉导管行脐静脉置管在新生儿复苏中的应用疗效佳,穿刺一次成功率高,注药顺畅,药液外渗少,导管脱出率低,值得临床推广。

Objective To investigate the epidural catheter in umbilical vein catheter applied effect in neonatal resuscitation. Methods From 2008 January to 2010 December in our hospital were born, resuscitation of neonatal umbilical vein in 36 cases as control group, in 2011 January to 2014 April in our hospital was born, neonatal umbilical vein resuscitation of 36 cases for the observation group. The control group using the traditional method of umbilical vein injection. The observation group was treated with epidural anesthesia catheter for umbilical venous catheter. To observe the one-time success rate of catheterization, injection of smooth, physic liquor exosmosis rate, emergence and the index of. Results In the observation group, epidural anesthesia catheter in umbilical vein catheter a success rate of 100% in the control group, one time success rate of puncture was 66.66%（24/36）. The comparison between the two groups, the observation group was significantly better than the control group, the difference was statistically significant （P〈0.05）. Two groups of children with the overall complication rate comparison, the observation group was significantly lower than the control group, the difference was statistically significant （P 〈 0.05）. Conclusion Epidural anesthesia catheter in umbilical vein catheter applied effect in neonatal resuscitation is good, a high success rate of puncture, injection and smooth, physic liquor exosmosis, catheter extrusion rate is low, is worth the clinical promotion.
