258例儿科住院患者合理使用抗菌药物调查分析

The Investigation and Analysis of Antimicrobial Drugs Used by 258 Pediatric Inpatients

目的调查儿科住院患者抗菌药物临床合理使用情况。方法随机抽样儿科住院患者297例,记录临床抗菌药物使用种类、时间、联合用药情况,并调查记录其是否进行药敏试验、病原学检查等,将所有调查项目及内容进行统计分析。结果 297例患者中,共有258例患者使用抗菌药物,占86.87%,其中,抗菌药物使用频率前5位分别为头孢西丁钠、头孢他啶、红霉素、头孢曲松钠、头孢哌哃钠舒巴他钠;有146例患者单独使用1种抗菌药物,占56.59%,另112例患者为两种抗菌药物联合使用,占43.41%,无1例患者采用3种抗菌药物及以上联合用药;使用抗菌药物前给予药敏检查及病原学检查的患者为116例,占44.96%;所有患者均给予静脉输注,部分患者静脉输注的同时给予口服。给予频率:22例患者为每日1次,占8.53%,236例患者为每日2次,占91.47%。结论我院儿科抗菌药物的使用比较合理,但也存在一些问题,如住院患者抗菌药物使用率偏高、抗菌药物使用起点过高、抗菌药物联用较多、部分抗菌药物的使用依据不足等,造成诸多用药安全隐患,临床应进一步加大管理力度,明确细菌感染相关依据,合理用药。

Objective to investigate the use of antimicrobial drugs by pediatric inpatients.Methods 297pediatric inpatients randomly were selected.The kinds,time and combination of the clinical use of antibacterial drugs were recorded.We investigeted sensitive test of the drug and etiological examination and carried out statistical analysis.Results 258 patients used antibacterial drugs,accounting for 86.87% of the 297 pediatric inpatients.The top five for frequency of using antimicrobial drugs were Cefoxitin Sodium,Ceftazidime,Erythromycin,Ceftriaxone Sodium and Sulbactam and Cefopcrazone.There were 146patients using an antibacterial drug,accounted for 56.59% and 112 patients using two kinds of antibacterial drugs,accounting for 43.41%.No patients used the combination of three kinds of antibacterial drugs.116 cases were given the drug sensitivity test and etiological examination before using antimicrobial drugs,accounting for 44.96%.All patients were given the intravenous infusion and some of patients were for oral drugs.22 patients were for one time a day,accounting for 8.53% and 236 patients for two times a day,accounting for 91.47%.Conclusions The use of antimicrobial drugs in Pediatrics of our hospital is reasonable,but some problems exist,such as high frequency for using antibacterial drugs in inpatients,high starting point of using antibacterial drugs,more combination of using antibacterial drugs and inadequate evidences for some drugs,which caused many problems in using drugs.It is important for the rational use of drugs to strengthen the management of using drugs and make clear evidences of bacterial infection.
