急性高危肺栓塞患者肌钙蛋白I检测的临床意义

Clinical Significance of Changes of Serum Cardial Troponin I in High Risk Patients with Acute Pulmonary Embolism

目的:探讨肌钙蛋白I(cardial troponin T,cTnI)评价急性高危肺栓塞(acutepulmonary embolism,APE)患者病情严重程度及其预测预后的价值。方法:选取2010年1月-2014年1月在阜阳市人民医院住院的确诊为高危APE患者90例,收集其cTnI检测结果、心脏多普勒指标和临床转归情况;根据临床转归,分为预后良好组和预后不良组,比较两组cTnI的差异,评价cTnI对APE高危患者的病情和预后的预测价值。结果:高危APE患者cTnI指标显著升高,经给予溶栓、抗凝等治疗5 d后随患者的病情好转,cTnI水平明显下降,治疗前后比较差异有统计学意义(P<0.01)。预后不良组治疗后cTnI水平明显高于预后良好组(P<0.01)。结论:cTnI水平能很好评估高危APE患者疾病严重性,治疗后cTnI水平具有预测高危APE患者发生临床不良事件的意义,为临床治疗和预后评估提供客观依据。

Objective：To evaluate the severity of acute pulmonary embolism（APE） patients by cardial troponin I （cTnI） and explore its prognostic value. Method：From January 2010 to January 2014 in Fuyang People’s Hospital,diagnosed high-risk APE patients with 90 cases were enrolled in this study.The cTnI, heart Doppler index and clinical outcomes were collected.According to the clinical outcome,patients of APE were divided into good prognosis group and poor prognosis group,we compared the differences between two groups of cTnI,predictive value of cTnI in high risk APE patients was evaluated.Result：In the patients with high risk APE,cTnI levels were significantly increased.After thrombolysis,anticoagulation with 5 days in the patients,the levels of cTnI were decreased, with a significant difference before and after treatment（P〈0.01）.In poor prognosis group the levels of cTnI after treatment were significantly higher than that in the good prognosis group（P〈0.01）.Conclusion：The levels of cTnI can assess severity of patients with high-risk APE.The levels of cTnI after treatment in the patients can predict the risk of adverse events and prognosis,and provide objective basis for clinical treatment and prognostic evaluation.
