软肝利胆汤对介入联合三维适形放疗肝癌患者生存质量的影响

Effect of Ruangan Lidan decoction on the quality of life in patients with hepatocellular carcinoma received three- dimensional conformal radiotherapy in joint

目的观察软肝利胆汤对介入联合三维适形放疗肝癌患者生存质量的影响,评价其在临床应用中的价值。方法将86例原发性肝癌患者随机分为治疗组与对照组,每组43例。对照组予介入联合三维适形放疗治疗,治疗组在介入治疗第1 d开始服用中药软肝利胆汤直至放疗治疗结束。2组患者分别于介入治疗前、介入后放疗前、放疗结束后采用Karnofsky功能状态评分(KPS)和肝癌患者生命质量测定量表(QOL-LC)对生存质量进行评价,并比较其变化。结果治疗组43例,脱落3例;对照组43例,脱落2例。2组病例脱落情况比较差异无统计学意义(P>0.05);治疗组在介入后放疗前及放疗后KPS评分改善程度明显优于对照组(P<0.05);介入治疗前,2组躯体功能、心理功能、症状/副作用、社会功能及总分评分比较差异无统计学意义(P>0.05);介入后放疗前,治疗组躯体功能、症状/副作用及总分评分均高于对照组(P<0.05),心理功能、社会功能评分相当(P>0.05);放疗结束后,治疗组躯体功能、心理功能、症状/副作用及总分评分均高于对照组(P<0.05),社会功能评分相当(P>0.05)。结论软肝利胆汤显著改善了介入联合三维适形放疗肝癌患者的生存质量,改善患者的躯体功能、临床症状及不良反应,改善患者的心理状态。

Objective To observe the effect of Ruangan Lidan decoction on the quality of life in patients with hepatocellular carcinoma received three- dimensional conformal radiotherapy in joint. Methods 86 cases of hepatocellular carcinoma were randomly divided into treatment group and control group,43 cases in each group. Control group received intervention combined with dimensional conformal radiotherapy. Treatment group received first started taking Chinese medicine on the basis of intervention combined with dimensional conformal radiotherapy until radiotherapy treatment. The quality of life was evaluated by KPS and QOL- LC before and after interventional therapy and before and after radiotherapy. Results There was no statistically significant difference between two groups on shedding（ P 〉 0. 05）. The improvement of KPS in treatment group was superior to that in control group after intervention and before and after radiotherapy（ P〈 0. 05）. Before intervention there was no difference between two groups on physical function,mental function,symptoms / side effects,social function and total scores rated（ P〉0. 05）.After intervention and before radiotherapy physical function and symptoms / side effects total scores in treatment group was higher than that in control group（ P 〈 0. 05）. There was no significant difference between two groups on the mental function and social function score（ P〉 0. 05）. After radiotherapy, physical function, mental function,symptoms / side effects and score ratings in the treatment group were higher than those in control group（ P〈 0. 05）. Conclusion Ruangan Lidan decoction can obviously improve the quality of life in patients with hepatocellular carcinoma received three- dimensional conformal radiotherapy in joint. It can improve physical function clinical symptoms and adverse reactions in patients improvingthe patient＇s mental state
