多种磁共振序列成像联合检测对首发抑郁症诊断价值研究

摘要 目的探讨首发抑郁症患者敏感脑结构在多种磁共振序列下的表现,并研究多种磁共振序列成像联合检测诊断首发抑郁症的价值。方法分别采用快速自旋回波三维序列(FLASH3d)、多体素(MVS)磁共振波谱检测序列(3D-CSI)及磁敏感加权序列(SWI)对20例首发抑郁症患者(观察组)和20例健康志愿者(对照组)的双侧海马进行体积形态成像、磁共振波谱检测及引流静脉成像。结果与对照组比较,观察组患者双侧海马体积明显缩小,差异有统计学意义(P<0.05);双侧海马谷氨酸盐混合物(Glx)/肌酸(Cr)及乙酰天门冬氨酸盐(NAA)/Cr降低,胆碱(Cho)/Cr升高,差异有统计学意义(P<0.05);肌醇(MI)/Cr变化不明显。双侧脑室下静脉最大直径(D)、长度(L)及分支数目(N)均减少,差异均有统计学意义(P<0.05)。结论首发抑郁症患者敏感脑区经多种磁共振序列检测均产生了变化,联合多种磁共振序列检测能更好地筛选出首发抑郁症患者,为临床防治提供了客观依据及帮助。 Objective To explore the brain manifestation of the first episode depression under multi-magnetic resonance(MR) sequence image,and research the diagnostic value of multi-MR sequence image on the first episode depression. Methods The FLASH3 d,(MVS)3D-CSI and susceptibility weighted imaging(SWI) sequence were adopted to detect the volume morphological imaging of bilateral hippocampus,magnetic resonance spectrum and draining vein imaging on 20 patients with first episode depression(observation group) and 20 healthy volunteers(control group). Results Compared with the control group,the volume of the bilateral hippocampus in the observation group was reduced obviously with statistically significant difference(P〈0.05);the values of Glx/Cr and NAA/Cr in bilateral hippocampus were reduced,the value of Cho/Cr was increased,and the differences between the two groups had statistical significance(P〈0.05);the changes of MI/Cr were not unobvious;the maximum diameter(D),length(L) and the branching number of the inferior ventricular vein were all reduced,and the difference between the two group was statistically significant(P〈0.05). Conclusion The sensitive encephalic region of the first episode depression has been changed after detection of multi-MR sequence image,the combination of multi-MR sequence detection can sieve the first episode depression better and provide the evidence and assistance for clinical prevention.
