鼻饲加味柴芍承气汤治疗急性胰腺炎40例

Nasal Feeding of Modified Chaishao Chengqi Decoction in Treating Acute Pancreatitis:a study of 40 cases

目的观察加味柴芍承气汤鼻饲治疗急性胰腺炎的临床疗效。方法将80例急性胰腺炎患者随机分成2组,2组均采取禁食、胃肠减压、静脉营养支持、抗感染、维持水电平衡及内环境稳定等综合治疗,对照组使用生长抑素(奥曲肽)0.1mg皮下注射抑制胰酶分泌,每8h1次,治疗7d为1个疗程。治疗组在对照组治疗基础上加用加味柴芍承气汤胃管鼻饲。观察治疗后患者的症状改善情况,评价其临床疗效。结果治疗组白细胞、血淀粉酶、尿淀粉酶、肠鸣音、排气、排便恢复时间及腹痛、腹胀消失时间和住院时间等各项指标均明显短于对照组(P<0.05);治疗组7d后总有效率明显优于对照组(P<0.05)。结论经胃管鼻饲加味柴芍承气汤治疗急性胰腺炎能改善患者临床症状,提高治疗效果。

OBJECTIVE To observe clinical effects of modified Chaishao Chengqi Decoction in treating acute pancreatitis by nasal feeding.METHODS Eighty patients with acute pancreatitis were randomly divided into two groups and integrated therapy of fasting,gastrointestinal decompression,intravenous nutritional support,anti-infection,maintain ing water balance and homeostasis were applied to the two groups.Patients in the control group received subcutaneous injection of 0.1mg Somatostatin（octreotide）to inhibit the pancreatic secretion 8hours a time and 7days was a course of treatment.On the basis of the above treatment,modified Chaishao Chengqi Decoction was applied to treatment group by nasal feeding to observe the alleviation of symptoms and assess the clinical effects.RESULTS The recovery time of white blood cells,serum amylase,urine amylase,bowel sounds,exhaust and the time required for the disappearance of abdominal pain and distention were much shorter than these of the control group（P〈0.05）.The effect in treatment group was better than that in the control group（P〈0.05）.CONCLUSION Nasal feeding of modified Chaishao Chengqi decoction is effective in treating acute pancreatitis.
