溴隐亭治疗利培酮致女性中度高催乳素血症的临床研究

Clinical study of Bromocriptine in treatment of moderate hyperprolactinemia caused by Risperidone

目的:探讨溴隐亭治疗利培酮所致的女性中度高催乳素血症的有效性,以及不引起精神症状波动的可能性。方法:根据ICD-10住院后确诊为精神分裂症的女性患者,且单一应用利培酮治疗的,待病情好转后,将治疗后出现中度高催乳素血症(100μg/L<催乳素<200μg/L)59例患者入组,均予以溴隐亭5mg/d治疗。4周后复查患者血清催乳素浓度;分别在第0、2、4周对所有入组对象行简明精神病评定量表(Brief Psychiatric Reacting Scale,BPRS)评定,验证精神症状与利培酮合并溴隐亭治疗疗效的相关性结果。结果:应用溴隐亭治疗中度高催乳素血症,其有效率98.33%(治疗后患者催乳素浓度低于100μg/L视为有效),用溴隐亭治疗后催乳素降低的有效性,有统计学意义〔t=22.166>t0.001(59),P<0.001〕,BPRS量表前后3次评定结果进行比较无显著差异。结论:本研究证实溴隐亭治疗利培酮所致女性中度高催乳素血症有实际临床应用价值,其有效性和安全性均非常可靠。

Objective：To discuss the efficacy of Bromocriptine in treatment of hyperprolactinemia cause by Risperidone and an-alyze the possibility of psychiatric symptom fluctuation for female patients with schizophrenia. Methods：According to the ICD-10, 59 female inpatients diagnosed as schizophrenia and got the mild hyperprolactinemia （100ug/ L〈prolactin〈200ug/ L） after the treatment with single Risperidone, when their statuses were improved, were treated with Bromocriptine at 5mg/ d, and their serum prolactin levels were reexamined four weeks after the treatment. All the inpatients were asked to finish the brief psychiatric reacting scale （BPRS） be-fore and 2 and 4 weeks after the treatment to test the correlation between the psychiatric symptom and the efficacy of Risperidone com-bined with Bromocriptine. Results： The effective rate of Bromocriptine in the treatment of hyperprolactinemia was 98. 33% （after the treatment, the prolactin level of less than 100μg/ L was valid）. There were significant differences by using Bromocriptines （t=22. 166＆gt;t0. 001（59）, P〈0. 001）. There were not significant differences in BPRS scores. Conclusions： The clinical study verifies that Bro-mocriptine in the treatment of moderate hyperprolactinemia caused by Risperidone has practical clinical values and reliable efficacy and safety.
