TK1、CA199及CA724联合检测在结直肠癌中的诊断价值

Clinical diagnostic value of combined detection of serum TK1, CAl99 and CA724 in colorectal cancer

摘要 目的探讨血清胸苷激酶1(TK1)、糖类抗原199(CA199)、糖类抗原724(CA724)联合检测对结直肠癌诊断的临床应用价值。方法采用电化学发光法对77例结直肠癌患者、61例结直肠良性病变患者和40例健康体检者的血清CA199、CA724含量进行检测,采用免疫印迹-增强化学发光法对各组血清TK1含量进行检测。结果结直肠癌组的TK1、CA199和CA724水平显著高于结直肠良性病变组患者和正常对照组(P均<0.05),结直肠良性病变患者的血清TK1、CA199和CA724水平显著高于正常对照组(P<0.05)。单项检测TK1对结直肠癌诊断的敏感度、特异度和准确度分别为40.3%、75.4%、55.8%,CA199为44.2%、88.5%、63.8%,CA724为45.5%、77.0%、59.4%;联合检测TK1、CA199和CA724为64.9%、68.9%、65.9%。结论 TK1、CA199和CA724水平对于结直肠癌的诊断、鉴别诊断具有重要的临床价值,三者联合检测可提高结直肠癌的检出率。 Objective To investigate diagnostic value of combined detection of TK1, CA199 and CA724 in colorectal cancer. Methods Serum levels of CAl99 and CA724 in 77 patients with colorectal cancer, 61 patients with benign colorectal diseases and 40 healthy controls were measured by western blotting-enhanced chemiluminescence method, and serum levels of TK1 were measured by enzyme immune chemiluminescence dot-blot assay. Results Serum levels of CAl99 and CA724 in patients with colorectal cancer were significantly higher than those in patients with benign colorectal diseases and healthy controls(P〈0.05). And those patients with benign colorectal diseases were significantly higher than those in healthy controls(P〈0.05). The sensitivity, specificity and accuracy of TK1 for the diagnosis of colorectal cancer were 40.3%, 75.4% and 55.8%, respectively; those of CA199 were 44.2%, 88.5% and 63.8%, respectively; CA724 were 45.5%,77.0% and 59.4%, respectively; combined detection of TK1, CA199 and CA724 were 64.9%, 68.9% and 65.9%, respectively. ConclusionThere are certain values of combined detection of TK1, CA199 and CA724 for the diagnosis of colorectal cancer, combined detection of TK1, CA199 and CA724 were helpful to increase the detection rate of colorectal cancer.
