2011—2012年云南省德宏州艾滋病病毒感染者病死率及死因分析

Fatality and Cause of Death among HIV-infected Individuals in Dehong Prefecture of Yunnan Province from 2011 to 2012

目的了解2011—2012年云南省德宏州的艾滋病病毒(HIV)感染者/艾滋病(AIDS)患者病死率及其死因。方法在国家"艾滋病综合防治数据信息管理系统"中下载2011—2012年现住址及户籍地均为云南省德宏州HIV/AIDS病例的历史卡片,对相关数据进行调查核实和统计分析。结果 2011年年初存活HIV/AIDS病例共7 178例,其中330例(4.6%)于2011年死亡;2011年当年新报告HIV感染者773例,其中46例(6.0%)于2011年当年死亡。2012年年初存活HIV/AIDS病例7 575例,其中326例(4.3%)于2012年死亡;2012年当年新报告HIV感染者598例,其中33例(5.0%)于2012年当年死亡。死亡的HIV/AIDS病例中,56.3%死因为艾滋病,18.4%死因为其他疾病(主要为肝脏疾病和心脑血管疾病),14.6%死于吸毒过量,7.2%死于伤害,2.2%死于自杀。结论德宏州2011—2012年HIV/AIDS病例病死率继续呈下降趋势,但死亡病例中近一半死于艾滋病以外的其他疾病和意外伤害等。为降低HIV/AIDS病例的病死率,应在加强抗病毒治疗的同时,加强肝脏疾病和心脑血管疾病的防控工作,减少吸毒过量和意外伤害造成的死亡。

OBJECTIVE To analyze the fatality and cause of death among HIV-infected individuals in Dehong Prefecture of Yunnan province during 2011--2012. METHODS Data about HIV/AIDS caseswho were registered as Dehong local residents or currently living in Dehong were extracted from the ＂Chinese National Comprehensive HIV/AIDS Prevention and Care Information System＂ and were analyzed. RESULTS At the beginning of 2011, there were 7 178 HIV/AIDS survivals in Dehong, of whom 330（4.6%） died in 2011. A total of 773 HIV/AIDS cases were newly reported in Dehong, of whom 46（6.0%） died in the year. At the beginning of 2012, there were 7 575 H1V/AIDS survivials, of whom 326（4.3%） died in 2012. A total of 598 HIV/AIDS cases were newly report- ed in 2012, of whom 33（5.0%） died in the year. Among dead HIV/AIDS cases, 56.3% died of AIDS, 18.4% die of other diseases especially liver diseases and brain and cardiovascular diseases, 14.6% died of over use of drugs, 7.2% died of injury, 2.2% died of suicide. CONCLUSIONS The fatality of HIV/AIDS cases continued to be decreasingfrom 2011 throughout 2012 in Dehong prefecture. However, approximately a half of the deaths were due to other events such as other diseases or injuries. Therefore, in order to re- duce the fatality among HIV/AIDS cases, more efforts are needed in prevention and control of other diseases such as liver or brain and cardiovascular diseases as well as the prevention and control of over use of drugs and injuries.
