体外受精—胚胎移植1742个周期临床分析

In-vitro Fertilization and Embryo Transfer:a clinical analysis of 1742 cycles

目的探讨体外受精—胚胎移植（IVF-ET）过程中影响妊娠结局的主要临床因素。方法选取2012年1—12月在合肥市某生殖中心进行IVF-ET的不孕症患者1 742例,查看IVF登记本,对不孕症妇女临床资料进行回顾性收集,应用SPSS软件进行统计分析。结果在1 742个周期中共获卵母细胞25 923个,受精19 909个,受精率76.80%;卵裂19 749个,卵裂率99.20%;临床妊娠623例,临床妊娠率35.76%。随着年龄增加,获卵数逐渐减少,各年龄组（≤35岁、36-40岁、〉40岁三组）的获卵数、受精率差异有显著性（P〈0.05）,临床妊娠率随年龄增大有下降趋势,〉40岁组的临床妊娠率为8.93%,显著低于其他组（P〈0.05）;随着获卵数增加,临床妊娠率呈上升趋势（P〈0.05）;两种不孕类型间临床妊娠率差异无统计学意义（P〉0.05）;与不孕年限≤10年组相比,不孕年限〉10年组的临床妊娠率显著下降,且两组间差异有统计学意义（P〈0.05）。结论年龄、不孕年限及获卵数是影响女性IVF-ET成功率的重要因素。

Objective To study the main clinical factors which impact pregnant outcome during in-vitro fertilization and embryo transfer（IVF-ET） process. Methods A total of 1 742 infertility patients undergoing IVF-ET in a reproductive center were enrolled in Hefei from Jan 2012 to Dec 2012,whose clinical data were retrospectively collected by consulting the IVF register and analyzed by SPSS software. Results In the 1 742 cycles,25 923 oocytes were collected,19 909 of which were fertilized with a fertilization rate of 76. 80%;among them,19 749 zygotes turned cleavage with a cleavage rate of 99.20%;623 cases got clinical pregnant with the pregnancy rate of35. 76%. With the increase of age,the number of retrieved oocytes decreased. The number of oocytes and fertilization rate of each age group（≤ 35,36 - 40, 40） had significant differences（P〈0. 05）. The clinical pregnancy rate had a negative relationship with age and clinical pregnancy rate of the age group of 40 was significantly lower than the other groups（P〈0. 05）;clinical pregnancy rate gradually increased with the number of retrieved oocytes（P〈0. 05）;infertility clinical pregnancy rates of the two types had no statistical significance（P〉0. 05）;the clinical pregnancy rate of the group which infertility duration ≤10 years was significantly lower than the group 〉10 years（P〈0. 05）. Conclusions Age,duration of infertility and the number of retrieved oocytes are prominent factors which affect female pregnancy rate in IVF-ET programs.
