2012—2013年我院门诊/住院药房退药原因及对策

Pharmacy drug repercussion analysis of a hospital in Anhui province from 2012 to 2013

目的对该院门诊/住院药房退药原因进行调查,以减少退药,促进医院合理用药。方法整理分析2012年1月—2013年12月该院门诊/住院药房退药处方2 680张,总结退药原因。结果退药主要原因是医嘱开错、更改医嘱、药物不良反应、患者拒用及药品发错等。结论提高医务人员的专业技能,合理用药,加强药品管理,减少退药情况的发生。

Objective To reduce drug withdrawal and promote the rational use of drugs in hospital through investigation of outpatient pharmacy / drug repercussion. Methods We summarized and analyzed the causes of 2 680 copies of prescribed drug withdrawal from January 2012 to December 2013 in Huaibei General Hospital for Mine Workers. Results The main causes of drug withdrawal were medication ordering errors,orders change,adverse drug reaction,patient refusal and wrong drug distribution etc. Conclusions Enhancing the medical staff＇s professional skills,reasonable medication and strengthening drug management can reduce the occurrence of drug withdrawal.
