高敏C反应蛋白和中性粒细胞及淋巴细胞比值与急性冠状动脉综合征的相关性研究 被引量:13

摘要 目的探讨血清高敏C反应蛋白(hs-CRP)和中性粒细胞及淋巴细胞比值(NLR)与急性冠状动脉综合征(ACS)及冠状动脉狭窄程度的关系。方法选择住院的ACS患者148例,分为不稳定性心绞痛(UAP)组60例,急性心肌梗死(AMI)组88例;另选择同期冠状动脉造影正常患者60例为对照组。检测血清hs-CRP、外周血白细胞计数及分类计数,并计算NLR,采用冠状动脉Gensini积分对冠状动脉狭窄程度进行评分。结果与对照组比较,AMI组hs-CRP[6.47(1.49,19.94)mg/L vs 0.96(0.42,1.45)mg/L]、白细胞计数(7.03±2.68)×109 vs(6.01±1.36)×109、NLR[3.21(2.34,5.29)vs 1.90(1.34,2.53)]明显升高(P<0.01),UAP组hs-CRP明显升高(P<0.01)。多因素logistic回归分析显示,hs-CRP、吸烟及血尿酸是UAP的独立危险因素,而hs-CRP、NLR和吸烟是AMI的独立危险因素。结论 hs-CRP和NLR与冠状动脉狭窄程度有关。 Objective To study the correlation of hs-CRP and neutrophils with lymphocyte ratio (NLR),acute coronary syndrome and coronary artery stenosis.Methods One hundred and fortyeight acute coronary syndrome patients admitted to our hospital were divided into unstable angina pectoris (UAP) group (n=60) and acute myocardial infarction (AMI) group (n=88),and 60 patients with normal coronary angiography served as a control group.Their serum high sensitive Creactive protein (hs-CRP) level was measured,their blood cells,NLR were calculated,and their coronary artery stenosis was scored according to the Gensini score.Results The serum hs-CRP level,the number of white blood cells and the NLR were significantly higher in AMI group than in control group [6.47(1.49,19.94)mg/L vs 0.96(0.42,1.45) mg/L,(7.03 ± 2.68) ×10^9 vs (6.01±1.36) × 10^9,3.21(2.34,5.29) vs 1.90(1.34,2.53),P〈0.01].The serum hs-CRP level was significantly higher in UAP group than in control group (P〈0.01).Multivariate logistic regression analysis showed that serum hs-CRP level,smoking,and serum uric acid level were the independent risk factors for UAP while serum hs-CRP level,NLR,and smoking were the independent risk factors for AMI.Conclusion The serum hs-CRP level and NLR are related with coronary artery stenosis.
